Nirvana is like the oil of an oil lamp, when it burns out, the flame of the lamp also goes out and disappears, meaning the end of life. Although as a medical worker it may be common to experience the withering of the flower of life, but should all the flowers of life be extinguished. Buddha said: saving a life is better than creating a seven-level pagoda. When faced with an extreme situation, what should we do and whether we will be able to make the flower of life bloom again. How to do it? The Buddha said: Although there are many healers, if they do not practice, they are the same as not hearing. From the bottom of our heart, we move with the kindness of a healer’s heart and act with the kindness of a healer’s art, without abandoning or giving up. I remember one day near noon in May 2012, two women rushed to the department with great anxiety to ask if their father could be saved. As a result, we learned that the patient was a patient with multiple intestinal fistulas with shock after pancreaticoduodenal surgery, who gave up and went home to wait for the last time because of the difficulty of treatment in the higher hospital. But the family was unwilling to give up like that, hoping for a miracle. Looking at their eyes full of expectation, we promised them. We decided to see the patient first before deciding whether to admit him or not. When we arrived at the patient’s home, listening to the sound of their borrowed negative pressure suction whimpering, looking at the patient’s open abdominal cavity with many grains of rice visible in the abdominal cavity, looking at his expectant eyes, seemingly unwilling to leave like this, there was still a gas left, firmly expressing the belief of not giving up and definitely wanting to live. When I saw this situation I seemed a little dumbfounded, although I had treated many patients with intestinal fistula in the past and had a lot of experience, but it was the first time I faced such an extreme patient, and the complexity and difficulty of its treatment was unimaginable. I am aware of the difficulties we will face in treating such patients in our basic hospitals, both in terms of the extent of the patient’s illness and our existing conditions, and more importantly, the fact that our hospitals simply did not have any experience and foundation for treatment before. However, the persistence and eagerness for life in his bones deeply moved us and organized 120 emergency vehicles to send the patient to our department. After admission to our department, we opened the abdominal cavity, placed double trocars for continuous flushing and drainage, total parenteral nutrition support, infection control and a series of other life-saving measures, and the patient’s condition was stabilized to some extent. I remember one day when we checked in he looked at us with his powerless and expectant eyes, seemingly disappointed and hoping for a quick recovery, and asked us, “Teacher, can I live?” . His wife and daughter were beside him, looking at us with tired and expectant eyes, and said with a choked voice, “Doctor, can he get better? Although his condition has improved, his stomach is still open, when will he get better?” . I said, “Your condition is slightly stable, there is still hope, the phoenix is nirvana, I believe you will be able to get better”. When the patient was admitted twelve days later, we placed the enteral nutrition tube under X-ray, the first placement did not turn the patient’s physiological arc, which was not very ideal, so we placed the tube again the next day, and through our efforts we succeeded. We knew that it was very painful for the patient who had a gastric tube placed, not to mention the need for repeated longer intubation to adjust the position of the tube. Later he told me how painful it was for him to have the nutrition tube placed, but he also understood that it was the most important and necessary part of his life, and he endured it silently all the time, leaving tears in his eyes when he watched the successful placement of the tube. Indeed, for patients with individual anastomotic fistulas after pancreaticoduodenal surgery, it is crucial and irreplaceable to be able to perform enteral nutritional support in relation to their ability to deliver nutritional substrates, promote anabolism and facilitate the growth of fistula granulation tissue. After about 120 days of hospitalization, the patient’s condition gradually stabilized and we gradually closed the abdominal cavity, the granulation around the fistula grew and the fistula gradually became smaller. Later we took various measures, protein glue blocking, modified continuous negative pressure pumping drainage (VAC) and other measures its open abdominal cavity gradually became smaller and the drainage fluid decreased. He actually got up on his own, went outside and started walking around, and even went home to rest on his own, seemingly telling people around him by his actions that he was alive. He told me he was so excited when he was able to get down and move around, and the happiness of being back on the ground was a feeling beyond words. Yes, the moment we saw him walking on the ground, we medical staff were very happy to see that our hard work had finally paid off. In 148 days, he was discharged from the hospital. The Buddha said: If you have a selfless heart, you have everything. Today the patient has been recovered for more than 1 year, and whenever they prompt this matter with their colleagues, it seems they are a bit afraid because its care volume is too big, the care process is too complicated, the care time is too long, and at the same time the new content of care needs constant follow-up. All this requires perseverance and firm belief, thorough, reasonable and meticulous care, a lot of energy and a lot of labor, and most importantly, a selfless love from the medical staff to create the miracle of life. For patients who need to experience the pain of the Phoenix Nirvana, we need to help them to build confidence, form the courage to break the boat and face the disease together, through good medical and nursing cooperation, to achieve the flower of life after the nirvana open again. For us, it seems that those former sufferings are turned into a flower fluttering in the heart, bringing a burst of fragrance to more people.
