Surgical treatment of patients with temporal lobe epilepsy with additional disorders

  My patient Xiao Lu had seizures for 5 years, which were not controlled by medication.  After a detailed evaluation in the first half of the year, the seizures were considered to originate near the right temporal lobe, which was completely MRI-negative for “non-focal epilepsy”, so intracranial electrode implantation was done to pinpoint the epileptogenic focus.  The evaluation of the intracranial electrode implantation went well and this implantation confirmed two epileptic origins: 1. right amygdala/hippocampus; 2. posterior temporal lobe and amygdala hippocampus together.  Functional cortical electrical stimulation examination did not reveal the presence of language function in the right temporal lobe, so an extended resection was performed. The patient is 2 days postoperative.
