U-shaped indentation of the neck usually indicates the presence of a cord winding, which is a common complication of pregnancy and statistically accounts for about 1/3 of pregnancies. Cord winding usually does not cause serious consequences and is not an indication for cesarean delivery. It is important to pay attention to fetal movements during pregnancy with cord winding and to monitor the fetal heart rate well, even remote monitoring is recommended. During labor, we should pay attention to the fetal heart changes. Due to the fetal head dropping and the umbilical cord pulling up, sometimes there is a transient fetal heart deceleration, which can be improved quickly through position change and oxygenation. If it does not improve, a cesarean section or forceps delivery may be required, but this is very rare.
