Gout often strikes suddenly at night or early in the morning, and in more than 50% of patients it starts in the metatarsophalangeal joints that connect the big toe to the ball of the foot, and manifests itself as joint pain, redness, swelling, and warmth. The pain may reach its peak within a few hours, but in most cases it resolves within a few days, and in some cases it lasts a little longer, but rarely more than 2 weeks.
Gout may recur if the triggers causing the attack or hyperuricemia do not improve; patients with chronic gout may also present with joint deformities and gout stones. The duration of gout in joints such as the big toe is prolonged in such patients.
