The plastic surgeon will make a plan based on the candidate’s personal aesthetic requirements, which will be fully exchanged with the candidate during the interview. Then the root (dorsum), tip, nostrils, columella, nasal wings and length of the nose will be treated in the following ways. (1) Surgery on the dorsum of the nose involves implanting materials to pad the dorsum of the nose, removing the raised hump, narrowing the wide dorsum of the nose, and correcting the crooked nose. (2) The treatment of the nasal tip, nostrils and nasal columella is the most complicated. It mainly involves cartilage transplantation, excision of hypertrophied nasal cartilage, and suturing to strengthen the original nasal tip cartilage. These techniques can elevate and reduce the tip of the nose, while precisely changing the aesthetic proportions of each subunit of the tip. (3) If the candidate’s nose is hypertrophic and flares out like a trumpet, the surgeon can remove part of the excess nasal flap and narrow the width between the two sides of the nose by retracting both sides of the nose. If the nostrils are retracted, causing the tip of the nose to resemble an arrow, then autologous cartilage transplantation can be used to improve the situation. (4) The standard length from the root of the nose to the tip of the nose should be about 1/3 of its facial length, and both too short and too long noses can be adjusted appropriately according to this ratio. Nowadays, doctors can build the candidate’s own cartilage into the septum between the two nostrils to lengthen the nose. Sophisticated aesthetic techniques may bring about dramatic facial changes, but their accompanying surgical costs, trauma, and risks increase accordingly. We recommend that candidates first define the shape of the nose they want; secondly, consider their vacation time (post-operative recovery period), their financial situation, and their ability to withstand the risks of surgery. Of course, the most important thing is to fully communicate with the surgeon during the pre-surgery consultation and listen to his/her advice, and finally choose the rhinoplasty surgery that suits your needs.
