Active prevention of breast cancer

      Breast cancer has become the number one malignant tumor among women and the “number one killer” threatening women’s health.  Every October is designated as the International Breast Cancer Awareness Month. Recently, in a press conference titled “Dialogue of Hope – Focus on Breast, Blossom Beautiful Life” co-sponsored by the Ministry of Health and the Chinese Anti-Cancer Association, Prof. Zhang Guangchao, Secretary General of the Chinese Anti-Cancer Association, introduced that the latest survey report released by the Ministry of Health shows that Breast cancer has become one of the fastest rising malignant tumors, with an increase of 96% in the past 30 years, and among the 8 types of cancer screening identified by the Ministry of Health, breast cancer screening is a priority. But Zhang Guangchao also said that although breast cancer has become the main malignant tumor threatening women’s health in China, early detection can achieve a 90% cure rate; through “breast-conserving” surgery, women’s quality of life can also be significantly improved.  Breast cancer kills 500,000 women worldwide every year. About 1.2 million women die of breast cancer worldwide every year, and 500,000 women die of breast cancer. In China, breast cancer has become one of the most common malignant tumors among women, ranking first in the mortality rate of female malignant tumors. According to the latest survey report released by the Ministry of Health, breast cancer has become one of the fastest rising malignant tumors, with an increase of 96% in the past 30 years, second only to lung cancer. In Shanghai, where the situation is most serious, the incidence rate reached 52.98/100,000; in Beijing, the incidence rate of breast cancer is also as high as 33.7/100,000, with an average annual growth rate even higher than that of European and American countries by 1~2 percentage points. At present, breast cancer in China has entered a period of rapid growth of 2% to 3% per year, and the peak incidence is mainly among women around 40 years old. Among the 8 types of cancer screening identified by the Ministry of Health, breast cancer screening is considered as a priority. Zhang Guangchao said that the publicity and education on early detection and treatment of breast cancer among the general public, especially the high-risk group, has quite positive significance for the treatment and prognosis of breast cancer.  Breast cancer screening is the most effective group prevention measure Breast cancer screening has been recognized in recent years as the most effective group prevention measure among malignant tumors to improve patient survival and reduce mortality, and the primary purpose of screening is to reduce subsequent cancer mortality through early detection of breast cancer. In order to detect breast cancer early, the Ministry of Health has launched a project called “National Breast Cancer Screening Program for Women” in March 2008. The one-year project, which is funded by the central government with 19.38 million yuan, will involve 530,000 women of the right age in 30 provinces (cities) and 53 counties (districts) across the country, with local trained physicians educating women of the right age about breast health and providing them with free, safe and professional breast examinations. Experts said that the screening program will further improve the screening program and explore a model of breast cancer screening suitable for Chinese conditions and biological characteristics of Chinese women, so that Chinese breast cancer patients can be detected and treated early, improving survival rates and reducing mortality.  ”Breast cancer prevention for women after 80s From Audrey Hepburn, Chen Xiaoxu to Ye Fan, breast cancer, the “killer of women”, continues to bring people shocking regret. Experts say that the high incidence of breast cancer is not terrible, what is terrible is not to detect it early. Since the first awareness campaign was launched in 2003, China has been conducting a large-scale scientific education campaign on breast cancer for five years. However, there are still many women who are not aware of the high incidence of breast cancer, especially some “post-80s” women who are confident that they are young and seem that breast cancer will not come to them.  In fact, this is a serious misunderstanding. Young women after 80s, who were born during the third population peak, often neglect their health due to high work pressure and fast-paced life; some women have excessive mental burden, poor sleep and endocrine disorder due to disorder in their life routine, which can be the cause of breast diseases. In addition, high protein, high fat diet, so that women menarche early menopause late, resulting in the body hormone always maintained at a high level. At the same time, environmental pollution and dietary pollution cause adverse hormonal changes in young women’s bodies, which also increase the risk of breast cancer in young women. In addition, the increase of breast cancer is also related to the delay of women’s childbirth in urban areas and the blind use of weight loss drugs and health supplements by some women.  More than 90% of the patients can be cured by diligent examination and early treatment. In order to save breast, examination should be diligent.  Monthly self-examination: pay attention to whether the size of breast is symmetrical, whether there are small nodules, whether there are changes in the skin of breast and the position of breast, and go to hospital for specialist examination in time if you find any suspicious changes. Annual physical examination: for women under 35 years old, it is recommended to have an annual breast ultrasound examination, the diagnosis can reach 85%; for women over 40 years old, it is recommended to include a molybdenum palladium X-ray examination program during the health checkup, the diagnosis rate can reach 90%. It is also reminded that general health screening is not a complete substitute for a breast specialist examination.  Do not take breast enlargement lightly, especially pathological enlargement with hyperplastic nodules.  This is because in the early stage of breast cancer, the mass is small and often indicates a hyperplastic nodule on high frequency ultrasound. If the ultrasound picture (which must be stored in high definition in the computer) is not carefully examined by a breast specialist, it may lead to a missed diagnosis of breast cancer. This type of breast cancer in the form of a hyperplastic nodule may start out as small as a sesame seed and cannot be palpated by a two-handed physical examination and is often not clearly visible on a mammogram. Minimally invasive breast surgery under high frequency ultrasound guidance is the best way to diagnose and treat this type of cancer.  In the past year, no less than 10 patients have been diagnosed with carcinoma in situ in our breast center after biopsy of suspicious hyperplastic nodules by mammography. The diagnosis of carcinoma in situ means that these 10 or so patients can be cured of breast cancer without the need for mastectomy and long-term survival without the need for radiotherapy, simply by removing the mass.  Minimally invasive breast surgery is not “surgery” in the traditional sense, it is only a treatment and is as painful as a muscle injection in the buttocks.  Minimally invasive breast surgery at our hospital is performed with the Johnson & Johnson vacuum-assisted minimally invasive breast rotation system (Figure 1), which was successfully developed and launched in 1999 and is capable of biopsy and complete excision of breast lesions in a precise, minimally invasive and safe manner. With more than 10 years of clinical experience and more than 2 million clinical procedures worldwide, the safety and reliability of this technology have been fully recognized by the world of breast surgery. In our breast surgery department, this technique has been carried out for nearly 10 years. With the continuous development of minimally invasive breast surgery, this surgery is no longer “surgery” in the traditional sense, not what we usually call “a knife to the breast”, it can only be considered as a kind of treatment, and the degree of pain is no different from that of a muscle injection to the buttocks. If a breast nodule is found on physical examination, in view of the various advantages of minimally invasive surgery, patients are advised not to wait for observation, because there are various uncertain risk factors in the waiting process. And the results will be better when the lesion is smaller and treated before it becomes malignant. It is as if a little broken tooth should be repaired by a dentist in time, without waiting for full decay before extracting the tooth. Treatment should be as early as possible.
