Some patients with brain tumors, after being informed of the need for surgical treatment, have serious fears based on hearsay due to the lack of understanding of craniotomy, resulting in clinical manifestations such as increased blood pressure and sleep disorders, which affect the postoperative recovery. Now, we introduce the operation process of craniotomy as an example of coronal incision right frontal craniotomy, which is often used in neurosurgery, so that we can have a sense of understanding of it. First of all, the doctor in charge needs to draw the marking line of the incision on the scalp, so that he can have a good idea when cutting the skin. The scalp needs to be covered with a sterile dressing after disinfection to prevent contaminants from falling on the scalp during the procedure. Local infiltration anesthesia is then required to be applied to the incision. The scalp is then incised and the skin margin is hemostatic using a special plastic scalp clip. The scalp is then separated outside the periosteum and turned aside to reveal the skull. This is the detached flap and the skull can be seen with the periosteum still covering the surface of the skull. The periosteum is cut on the surface of the skull that needs to be opened and turned to one side. This is an important tool in craniotomy – the pneumatic craniotomy drill. Here is the surgeon making the hole in the skull. Once the hole is made, the surgeon will insert the high speed cutter-mill into the hole and mill down the bone flap. This is what the skull looks like when it is opened, with the brain tissue tightly protected by the dura mater. By cutting further through the dura, the brain tissue can be seen and the surgeons can complete various surgical operations. Although there are so many steps in opening the skull, bleeding is usually minimal, and this is the reservoir for recovering blood. After the tumor is removed, the dura is tightly sutured and the skull is fixed back in place with the application of titanium clips. Now, even the periosteum is sutured. Finally, the scalp clips are removed and the scalp is sutured. With the general knowledge of craniotomy, it will be found that there is not much mystery in neurosurgery, and patients and family members have to fight side by side with doctors for an early victory over the disease.
