Cold or hot compresses?

  For sudden onset of pain in osteoarthritis of the knee, the first thing that comes to mind in the traditional Chinese approach to autonomy is hot compresses, the Chinese philosophy is to invigorate the blood and promote blood circulation, but there is some problem here, some of the pain is indeed relieved but continues the next day, and the other part is that the pain worsens at that time.  Here comes the understanding of acute inflammation. Acute inflammation is characterized by redness, swelling, and pain, i.e., inflammation consisting mainly of a reaction of the vascular system. The local blood vessels dilate and inflammatory and pain-causing factors exude into the tissues. Acute inflammation is like a fire that should be put out, not heated.  Cold and heat, the two opposites of physical therapy, may seem like a somewhat difficult choice to make, but in reality it is not difficult. Cold will cause vasoconstriction, slow muscle blood flow, and prolonged cold will lead to muscle spasm, but it can stop bleeding, eliminate synovial inflammation, and promote the recovery and reconstruction of collagen tissue within cartilage and tendons; heat will lead to vasodilation and congestion of muscle synovial tissue, which can relieve muscle spasm, but will aggravate inflammation and bleeding, especially in the acute phase of inflammation in arthritis, and overheating will lead to aggravation of inflammation.  Here are two special examples of the bad effects of hot compresses. One, a young man fell to the ground from a high place, slight pain in the right heel, the village doctor gave hot wine contusion and kneading, came to the clinic with full lower extremity swelling, subcutaneous bruising, X-ray showed heel fracture, no doubt the hot compress led to bleeding, and bleeding is very large, the consultation time is any later, may occur fascial compartment syndrome, leading to serious consequences. Second, old light female, severe osteoarthritis of the right knee, recurrent for many years, severe pain in the knee joint after a certain cold, and even more swelling in the right knee with hot compresses such as cold pain relief on her own, came to the outpatient joint puncture, and I did not expect the puncture fluid to be bloody! This shows that the synovial membrane in the joint during the acute phase is congested and fragile, and once it is hot, it breaks and bleeds.  This shows that in the vast majority of acute stage injury cases and at least half of the chronic strain cases cold compresses are not wrong, the opposite may be bad; only in half of the chronic pain, hot compresses in the muscle abundance is good, it can be seen that heat compresses for chronic stage pain, including hot water, heat therapy, wax therapy, herbal fumigation, mastering the time, will significantly improve local blood circulation, conducive to the discharge of inflammatory factors, and It plays a very good therapeutic role.  Therefore, there is an experience that for middle-aged and elderly people with osteoarthritis of the knee during the attack period, that is, when the joint is swollen than usual, the temperature is slightly higher and the pain is severe, suggesting that it may be in the attack period of osteoarthritis, it is better to do nothing and rest when the effect of cold or hot compresses is not clear, and then seek help from a doctor for diagnosis and medication.
