Why are there so many patients with lower extremity varicose veins and so few visits?

Varicose veins of the lower extremities is a common disease of vascular surgery, occurring in long standing, weight-bearing workers and pregnant women, and there is a family genetic predisposition, the common clinical manifestations of superficial veins of the lower extremities are tortuous expansion of bulging, twisted into a mass of earthworm-like, not only affecting the aesthetic appearance, but also the patient often feel soreness and discomfort in the lower extremities, feeling of fatigue, standing for a long time, ankle and dorsum of the foot edema, the skin color of the inner ankle of the lower leg pigmentation, severe cases with eczema, prolonged skin ulcers. In severe cases, the lower limbs are accompanied by eczema, prolonged skin ulcers, tortuous superficial veins are prone to thrombosis due to stagnation of blood flow, resulting in thrombophlebitis, localized redness, swelling, pain and hardening of the skin, and in a few cases of severe varicose disease, there is also a risk of frictional rupture of the varicose mass and hemorrhage. Epidemiologic results show that the incidence of varicose veins of the lower extremities in people over 15 years of age in China is 7.9%-8.6%, and over 45 years of age is 16.4%, so the total number of patients in China is at least more than 80 million. 1. There are many patients with varicose veins, but why not more? Because many patients just find the blood vessels in their legs are enlarged, earthworm-shaped, and can disappear when they lie down, but there is no pain, or the pain is not heavy, can be tolerated, and does not affect their daily life, so they feel that there is no need to diagnose and treat. Many patients with varicose veins in our country have very serious varicose veins and develop dermatitis, eczema, hyperpigmentation, thrombophlebitis, skin ulcers or hemorrhage before they come to the clinic. 2. Why some patients have not been investigated for many years while others have been detected with the same varicose veins? This phenomenon is related to the presence or absence of complications, the degree of sensation and tolerance of individuals. People with acute sensation and poor tolerance to pain, in the beginning of superficial vein expansion stage, may be due to the stimulation of the vein peripheral membrane receptors, that is to say, there is a feeling of acidity, discomfort and pain, obvious when standing, walking, and disappear when lying down. The course of the disease continues to develop, the venous hypertension and stasis become more and more serious, there may be swelling of the calf and trophic changes of the skin in the boot area: skin atrophy, desquamation, itching, hyperpigmentation, hardening of the skin and subcutaneous tissues, and even the formation of eczema and skin ulcers. 3. Can wearing compression stockings treat varicose veins? In non-surgical methods, the main thing is to wear medical compression stockings, or play elastic bandage, so that the varicose veins are in the state of shriveling, and to elevate the affected limbs, take proper rests, and avoid standing for too long. However, this can only stop the progress of the disease and cannot make the varicose veins disappear. Compression stockings are suitable for: ① the degree of varicose is very light, the range is very small; ② pregnant women due to the growing uterus after pregnancy caused by compression of the deep veins, may disappear after the night; ③ heart, lungs, brain and other important organs have serious diseases that can not tolerate surgery. 4.What is the most effective method? Surgical therapy is the most fundamental and effective method to deal with varicose veins of the lower limbs. As long as there are no contraindications, such as severe cardiopulmonary diseases and very poor surgical tolerance, surgical methods should be chosen. At present, there are many kinds of methods for varicose veins of the lower limbs: high ligation of the great saphenous vein, modified minimally invasive stripping, rotary cutting; minimally invasive non-stripping methods include: endovenous radiofrequency ablation of the great saphenous vein, endovenous laser occlusion, electrocoagulation occlusion, and sclerosing agent injection. Most of the patients with varicose veins of the lower limbs who come to the clinic in China have more serious varicose veins and more complicated conditions, and it is generally difficult to achieve satisfactory therapeutic effect by simply applying a minimally invasive method. Therefore, the treatment of varicose veins of the lower limbs should be comprehensive, and several minimally invasive techniques should be jointly applied to complement each other’s strengths and advantages, and the modified peeling procedure combined with minimally invasive treatment of varicose veins of the lower limbs can obtain the treatment effect of the traditional procedure, and can also be effective and effective. In this way, both the traditional treatment effect and the minimally invasive way can be obtained to avoid the problems of excessive trauma, postoperative pain and more postoperative complications. This will make the patients with varicose veins of the lower limbs suffer less pain, recover faster, and maintain the aesthetic appearance of the legs to get a satisfactory treatment result.
