Chocolate cysts on MRI



The examination of chocolate cysts by MRI has various manifestations, but generally there are abnormal manifestations in T1WI and T2WI.

Through MRI examination of chocolate cysts, it can be found that the cysts as a whole are in the shape of a sharp angle, due to the repeated bleeding of chocolate cysts, which will gradually increase the size of the cysts and will form adhesions with the surrounding tissues. In the T1WI signal increase, T2WI signal decrease, appear shadow, can be manifested as completely homogeneous shadow, also can be manifested as focal patchy or striated shadow.

However, it should be noted that chocolate cysts are hormone-dependent tumors, which will change differently at different menstrual periods, and thus show more complex signals and morphology in MRI, and are easily confused with corpus luteum cysts and ovarian follicular cysts.
