Conservative therapy is the basic method of treating lumbar disc herniation. The purpose of treatment is to partially absorb or retract the herniated disc nucleus pulposus tissue and eliminate the compression and irritation of nerve roots, so as to relieve or eliminate pain symptoms. These include: bed rest, physical therapy (massage, etc.), Chinese medicine (including acupuncture), traction, local closed drug injection, etc. Bed rest is one of the most important measures in conservative treatment, which can reduce the pressure on the intervertebral disc and spinal canal, help to relieve the pressure on the nerve root and cauda equina nerve, and reduce the edema of the nerve root, which is very effective for the first attack and those with mild symptoms and signs. When the first attack of symptoms, should be “absolute” bed rest, the word “absolute” means: eating, urinating and defecating should not get out of bed or sit up, so as to receive good results. Of course, too much time in bed and lack of necessary muscle exercise will lead to muscle atrophy, so after the symptoms and signs are relieved, appropriate muscle exercise should be carried out. It should also be noted that the bed should not be too soft or too hard, and should be wide enough to facilitate the patient’s turning and to prevent bed sores; it is best to insist on being in bed for a period of time, usually 3 weeks or more; the bed position can be supine, lateral, prone and kneeling, etc., mainly to feel comfortable.
