How to cure frozen shoulder for more than 20 years?

  Two months ago, Ms. Wang in Putuo District was referred to me by a patient I had treated and told me about her tortuous experience of seeking medical treatment for frozen shoulder, which had been tormenting her for more than 20 years. 60-year-old Ms. Wang developed pain in her left shoulder for no reason more than 20 years ago, and her shoulder joint movement was limited, and the pain was unbearable when she exerted force or increased her activity, and the pain was so heavy at night that she often had difficulty sleeping. In the past three years, he has been visiting many Chinese and Western medicine hospitals in Shanghai, and the treatment he received was considered difficult and ineffective. After examining the patient, Dong Zhangli from the pain department of Shanghai Deji Hospital found that the patient had obvious pressure pain around the shoulder joint, atrophy of many muscles around the shoulder to varying degrees, and significant limitation of movement of the left shoulder, which prevented him from combing his hair and tying his trouser belt, thus affecting his daily life significantly. After detailed questioning and examination of the shoulder, it was determined that the aseptic inflammation of the myofascia and tendons around the shoulder joint had not been eliminated, and that there were severe ligament adhesions and contractures. Therefore, comprehensive treatment was given to eliminate the inflammation, release the spasm, loosen the adhesions and functional rehabilitation. After one week, the pain was significantly relieved, the pain disappeared at night and the shoulder joint movement was significantly improved. After one month, the pain basically disappeared, the shoulder joint activity was close to the normal range, and the influence of life was basically eliminated, and the patient was very satisfied. One and a half months later, the pain in the left shoulder disappeared, the activity returned to normal and the frozen shoulder was cured.  So how was Ms. Wang’s frozen shoulder, which had been tormenting her for more than 20 years, cured? Why did she have no significant effect even after arthroscopic surgery?  Let’s first understand the cause and pathogenesis of frozen shoulder. Frozen shoulder is also known as periarthritis of the shoulder joint, commonly known as frozen shoulder and fifty shoulder. It is a chronic aseptic inflammation of the shoulder capsule and its surrounding ligaments, tendons and bursa, with gradual pain in the shoulder, heavy at night, gradually aggravated, and limited shoulder joint function and increasingly aggravated. There may be widespread pressure pain in the shoulder joint that radiates to the neck and elbow, and there may be varying degrees of periacetabular muscle atrophy. The prevalence of this disease is around 50 years old, with a slightly higher incidence in women than in men, and is most often caused by chronic strain in manual workers. Although it is a common disease, if it is not treated effectively, it may seriously affect the functional activities of the shoulder joint, and if it does not heal for a long time, it will seriously affect life and work.  For Ms. Wang, the reason why so many treatments were used, including arthroscopic surgery, but no significant results were achieved was because there was no detailed analysis and assessment of her disease, its function and etiology, and only a single treatment was taken for a certain aspect, especially no functional assessment and rehabilitation. She was even told early on that she could heal on her own without treatment, and if she did not heal for a long period of time, she was considered a difficult disease, and there were two extremes.  Therefore, even though frozen shoulder is a common disease, if it is not treated promptly and effectively, it can remain untreated for a long time, seriously affecting the patient’s life and work and causing severe pain.
