In recent years, thyroid nodular disease has received increasing attention, with 19% to 67% of nodules detected during ultrasound and other imaging examinations; the incidence of thyroid cancer among nodules is about 5% to 15%, and the detection rate of thyroid nodules in women is 2.45 times higher than that in men. Traditional thyroid surgery leaves obvious surgical scars on the neck, causing a great psychological burden to female patients. With the development of laparoscopic technology in many fields, the use of laparoscopic technology in thyroid field has a certain foundation and conditions. On the basis of more than 100 cases of laparoscopic-assisted small incision thyroid surgery in the neck, the Laparoscopic Surgery Department of the First Affiliated Hospital of Dalian Medical University has recently completed a subtotal thyroidectomy by the transthoracic breast route (with no scar in the neck), which has been a boon to female patients who love to look beautiful. The patient is a 33-year-old woman, who had found thyroid nodules for many years and refused surgical treatment due to the fear of surgical scars. Recently, the nodules increased significantly, with a maximum diameter of 3.8 cm and accompanied by symptoms of compression, and she was admitted to the hospital, requesting for cosmetic surgery, which was basically excluded from the malignant possibilities in consultation with the Department of Ultrasonography, and the surgery was completed successfully by taking small incisions of 0.5-1.0 M in the bilateral paramedian areola and the right breast without any post-operative complications. There was no complication after surgery, and the appearance of the neck and anterior chest did not change from the preoperative period, and the patient was very satisfied. At present, thyroid surgery via the transthoracic breast route is mainly for patients with benign thyroid lesions, early thyroid cancer without lymph node metastasis, and patients with cosmetic requirements.
