Lumbar disc herniation is one of the common clinical diseases, belongs to the category of “paralysis and lumbago” in Chinese medicine, often caused by both trauma and internal causes, with more incidence in young adults. The disease is often caused by trauma or by careless labor or exercise, or by a defective development of the intervertebral disc, or by wind, cold or dampness, or by degenerative changes in the lumbar intervertebral disc due to acute or chronic injury, resulting in inflammation of the fibromuscular tissue, surrounding vascular nerves, edema, or even rupture of the fibrous ring, nucleus pulposus or protrusion, stimulation and compression of the spinal nerve roots. This causes unilateral or bilateral sciatica, or even limited movement of the lower limbs. As modern life becomes more and more urbanized, people’s life and work pressure become more and more stressful and the level of medical treatment continues to develop, the number of patients with lumbar disc herniation is increasing. In general, more and more “office workers” are suffering from this disease due to work and habits. At present, Western medicine treatment methods for herniated discs include nucleus pulposus, ozone therapy, surgery, etc., but no single method of treatment can be said to be curative. Therefore, how to use the advantages of Chinese medicine to relieve the sufferings of the majority of patients is worth exploring for our Chinese medicine students. I will talk about the treatment of this disease in Chinese medicine as far as I know. As early as in the Yellow Emperor’s Classic of Internal Medicine. Su Wen”, the forty-first book – stabbing back pain, there is a record of similar to this disease, and also gives a detailed classification and treatment methods. For example: “The foot sun pulse makes the lumbar pain, leading to the collar and back of the coccyx like heavy, stab its Qiezhong. The Sun proper bleeds, and no blood is seen in spring.” “The veins of the sympathetic yin cause lumbar pain, and the waist is like the string of a crossbow. Stabbing the pulse of syncope, in the Y heel, outside the abdomen of the fish, follow the tired, is stabbed. Its disease makes people good at talking silently and not wise, stabbing the three _.” It can be seen that although in ancient times there was no modern diagnostic equipment such as x-ray existed. However, the symptoms of lumbar disc herniation have always existed. Therefore, Chinese medicine also has a wealth of experience in treating this disease. Broadly speaking, it is divided into three aspects: acupuncture treatment, Chinese herbal medicine treatment, and guiding treatment. In view of the limited space and the fact that I don’t know much about acupuncture and Chinese herbal medicine, I would like to focus on my views on the treatment of this disease. Chinese medicine generally believes that herniated lumbar discs are mostly caused by wind, cold, dampness or trauma. The Yellow Emperor’s Classic of Internal Medicine. The book of “Paralysis” says that “wind, cold, and dampness are mixed together and become paralysis ……”, and that “paralysis is also closed, which means that it is blocked”. The “Treatise on the Origin of Diseases. The “pain in the waist and feet” says: “kidney deficiency, by the wind and evil for also, labor injury is kidney deficiency, deficiency is subject to wind and cold, wind and cold and righteousness fight, so the waist and feet pain.” Therefore, Chinese medicine treatment of this disease, mostly from the wind, cold, damp, stasis, deficiency treatment, the initial stage is appropriate to activate the blood to relax the tendons, remit the paralysis, commonly used to invigorate the blood and Tongluo a class of formula plus or minus. If the disease is prolonged, the body is deficient, so the treatment is to nourish the liver and kidney, to promote paralysis and pass the ligaments, often used to nourish the kidney and strengthen the bones of a formula plus or minus. Such as: Douwuxiaosheng Tang. But I personally think that this dialectic is far from enough. Because Chinese medicine focuses on dialectical treatment, it is necessary to determine the correct treatment principle and treatment method according to the patient’s own clinical manifestations. If we use the formula of blood activation and blood circulation to treat all of them, this is called treating the disease and losing the essence of TCM treatment. Inevitably, we will not get very good results. In my opinion, “The Golden Book of Medicine. I think the song in the chapter of “Miscellaneous Diseases Heart Method Essentials” summarizes the causes of this disease very well. It classifies low back pain into nine categories: kidney deficiency, wind, cold, dampness, phlegm, qi stagnation, blood stasis, dampness and heat, and flash and contusion. In this way, nine different principles of treatment are identified. That is, tonifying the kidney, dispelling wind, dispersing cold, resolving dampness, expelling phlegm, regulating qi, invigorating blood to dispel stasis, clearing heat and dampness, and maintaining exercise. It also gave many good basic prescriptions for treatment, such as Qiangwu Shengshu Tang, Ankuan Wan, Tongqi San and Cangbai San. He also believes that if a patient has “sudden redness and darkness in the face, it is an evidence of difficulty in treating the heart and kidney.” Of course, in clinical practice, patients rarely have just one simple cause, but often have multiple causes mixed together, with a mixture of cold and heat, and a combination of deficiency and reality. Therefore, the specific individualized treatment plan requires careful dialectic and flexible formulae. In addition, the comprehensive treatment of TCM should be given full play. In addition to internal tonics, it should also be combined with external application, acupuncture and moxibustion, and tui-na. For example, small acupuncture and bone setting in Chinese medicine have very special efficacy in this regard. In addition, for this disease, daily living attention and health care exercise also occupy a certain importance. As the saying goes, “if the righteousness is stored inside, the evil cannot dry up”. With a good work and rest schedule and adequate exercise, you can naturally reduce the occurrence of this disease. After all, prevention is better than cure. This is the same point of view as Chinese medicine, which says “treating the disease before it happens”. In summary, although there is no good cure for this disease. However, the comprehensive treatment of TCM is far safer and more effective than the Western treatment methods, and is also more acceptable to the Chinese people. Therefore, we should continue to study in depth the details of the integrated treatment process of TCM. We should make the various therapies work together more effectively. We believe that one day we will overcome this disease.
