Abstract: This article describes an elderly patient with varicose veins for many years, who thought she was painless and itch-free and had not been treated for many years, and suddenly developed skin pigmentation six months ago. After admission to the hospital with a clear diagnosis, saphenous vein stripping + foam sclerotherapy was used. The operation went smoothly, and the incision healed well with no obvious redness or exudation after 3 days of postoperative observation, and no obvious complaints of discomfort and no varicose veins were seen on follow-up. Initial consultation In October 2021, an 80-year-old male patient came to the department with “30 years of varicose veins in the right lower extremity and six months of foot pigmentation”, which appeared 30 years ago when he was a soldier in the army. He came to the hospital for further treatment. Recently, he had been eating and sleeping well, and his bowel movements were as usual, with no significant weight loss. The patient had a history of hypertension for more than 10 years, with good blood pressure control and no history of diabetes, heart disease or cerebrovascular disease. Physical examination: local varicose veins were seen in the right lower extremity, with the right calf being the heaviest, and local dark purple skin color was seen at the inner ankle of the right foot, with slightly high skin temperature and local light pressure pain. Treatment After admission, the relevant examination was completed to assess the condition. Venous ultrasound of the lower limbs indicated that the right calf segment had saphenous varicose vein, and the deep veins did not show any obvious abnormality. Considering that the patient was an elderly male with a history of hypertension, the minimally invasive procedure of radiofrequency closure + foam sclerotherapy was most appropriate, but because the patient was thin and the main trunk of the saphenous vein in the thigh segment was too close to the skin (<2cm), in order to avoid scalding the skin with the radiofrequency catheter, the patient and his family were advised to retreat to the second best option of sleeve saphenous vein stripping + foam sclerotherapy. This approach requires only a few small 1-2 cm incisions, is less invasive, can be done with local anesthesia, and is a good patient experience. The patient’s surgical operation went smoothly and the patient recovered well after surgery. The incision healed well after 3 days of postoperative observation, and the compression stocking was replaced to further promote venous return and discharge was scheduled. III. Treatment effect The patient was given incision change 3 days after the operation, and the incision site healed well without redness, swelling and exudation. The patient was given a dressing change for the incision, the elastic bandage was removed, and the medical compression stocking was replaced to continue to promote venous reflux. 1 month later, the patient was followed up in the outpatient clinic, and six months later, the patient was reviewed and it was seen that the varicose veins were cured and there were no obvious varicose vessels and no recurrence, and he was satisfied with the treatment effect. IV. Precautions The patient was sincerely happy for the patient that the varicose veins disappeared after the surgery. In addition, the following matters need to be noted: 1, varicose vein stripping after surgery will appear some local skin bruising, which is caused by subcutaneous bleeding, in most cases can dissipate on their own, the correct post-operative binding elastic bandage or correct wearing of compression stockings will reduce the occurrence of this situation; 2, varicose veins after surgery is recommended to go to the ground early, such as not to go to the ground, post-operative hypercoagulation plus blood stasis will induce deep vein thrombosis. Therefore, for patients after varicose vein surgery, it is recommended to get out of bed early if there is no special anesthesia requirement. V. Personal insight Varicose veins of the lower extremities are the most common type of venous disease of the lower extremities, which are asymptomatic in the early stages but slowly develop into tortuous dilatation of blood vessels, venous tumors, skin pigmentation, and even serious complications such as phlebitis, ulcers, bleeding, and thrombosis. The treatment of varicose veins is not simply judged by the severity of its symptoms, even if the patient has no obvious symptoms, but the protruding blood vessels are obvious, and even pigmentation, should be actively dealt with, otherwise the further development of serious complications, it will not be able to return to normal conditions.
