Why is topical medicine important?

  I just sent off a patient some time ago, and the patient got better and was discharged, but I couldn’t feel calm for a long time. The patient is a 36-year-old young woman who injured her right calf when she went out to play rafting, and because of the pain, the patient applied “orthopedic water” topically, but the symptoms were not relieved, the whole right calf was in pain, and the area where the “orthopedic water” was applied was red, swollen and hot. At the suggestion of a friend, the patient adopted certain prescriptions, using Chinese herbs (specific unknown) to fumigate and soak, which became a big problem, the entire right lower leg was red and swollen, and gradually appeared large and small blisters, pain is unbearable, until this time, the patient decided to seek help from doctors.  The patient finally came to us after he had been transferred to many hospitals. He had checked the blood picture with more than 20,000 white blood cells, elevated liver enzymes, elevated cardiac enzymes, abnormal coagulation function, and some dense petechiae and bleeding spots on his body, and was given a diagnosis of “multi-organ dysfunction” by the emergency room. The surgeon did not rule out the possibility of amputation if necessary, and the patient was sent to the dermatology department after several changes. As soon as I saw this patient, I was shocked, his expression was painful, and his right calf was wrapped with layers and layers of cotton pads and gauze, very thick, and drug exudates were mixed together, as if he was a casualty who had just come from the battlefield, so I thought there was another troublesome patient. The problem was divided into redness, swelling, pain, blistering, erosion, ulceration and necrosis, and a series of measures were given in an orderly manner, including extraction of large blisters, elevation of the affected limb, simple wet dressing with saline gauze, anti-infection, diuretic and anti-swelling, and hormonal and anti-allergic treatment. 1-2 weeks later, the redness and swelling subsided, and the new epidermis grew back after skin desquamation, leaving only Today, I heard that the ulcer has been recovered by a local skin graft.  The story is finished, but the thinking doesn’t stop there. It all started from the patient’s disregard and ignorance of topical skin treatment, which almost led to the tragedy of amputation. In my normal practice, I often encounter similar patients: ① using some unknown powder to sprinkle on eczema oozing, burn oozing; ② applying some red or blue salve when the skin is broken; ③ using “safflower oil, bruise wine, pain relief cream, wind repellent oil” or other herbal compresses on their own.  And then the allergic symptoms, light itchy red spots, heavy blisters and ulcers, and thick powder, scabs or other medicinal water color often increases the difficulty of rash examination and judgment, masking the disease. In medical terms, we call this type of condition “contact dermatitis”. These patients are mostly old ladies, of course, there are also some young parents, very superstitious about the above oil and wine prescriptions, but also strongly claim to be purchased from abroad or Hong Kong and Macao or a famous old Chinese doctor prescribed.  Speaking of which, I can’t help but think of a doctor in Guangzhou who published on Weibo a case of abuse of “xxxx white medicine” that led to an aggravation of the rash, but unfortunately had some unpleasant disputes with the “xxxx white medicine” as a result. I am not saying that the above mentioned drugs should not be used or their efficacy should be doubted, but they should not be abused and you should not think that it is okay to just rub them in. All doctors in the dermatology industry know that there are basic principles for topical dermatological drugs, when to apply wet, when to use powder, when to use cream, when to use ointment, very careful, the slightest carelessness may lead to new problems.  Therefore, do not take the skin problems improper problems, do not take the topical drugs not seriously, there are situations to find a doctor, there is a discussion, reasonable use of drugs, while the treatment of observation is the best way.
