Cervical medullary epidural lesions are one of the clinical manifestations of spinal cord compression. Medullary compression refers to a group of conditions in which the spinal cord, spinal nerve roots and their supplying vessels are compressed by lesions of various natures. Examination items for cervical medullary epidural lesions: 1. Radial periosteal reflex The radial periosteal reflex is the reflex of percussion of the radial styloid process, flexion of the elbow, flexion of the fingers, and rotation of the elbow joint forward. The radial membrane reflex is the reflex of tapping the radial styloid process, flexing the elbow, flexing the fingers, and rotating the elbow joint forward. The inverted radial reflex is a special reflex manifestation. As the contraction force of the active muscle becomes smaller due to paralysis, the stimulus is transmitted to the anterior horn of the spinal cord and then diffuses, causing the contraction of the antagonist muscle at the same time; or due to the paralysis of the active muscle, causing the pull reflex of the antagonist muscle. 2, Hoffmann’s sign Hoffmann’s (Hoffmann) sign belongs to the pathological reflex. The examiner holds the patient’s middle finger with the index and middle fingers of the right hand and makes the wrist joint dorsiflexed, while the other fingers are in a natural relaxed semi-flexed state, then the examiner quickly scrapes the nail of the patient’s middle finger with the thumb, if the palmar flexion movement of the other fingers appears, it is positive for Hoffmann’s sign. 3.Pathological reflex examination Abnormal reflexes that only appear when the central nervous system is damaged are called pathological reflexes. The presence of pathological reflexes is a confirmation of damage to the pyramidal tract, which means that the pyramidal tract has lost its inhibitory function on the brainstem and spinal cord. In adults, the above reflexes are pathological. Pathological reflexes are commonly used in clinical examination: Babinski, Oppenheim, Gordon, Chaddock, etc.
