Beginning at age 40, we gradually experience the pain of presbyopia. Many years ago I helped a 46-year-old female patient who had cataract surgery on one eye. I implanted a multifocal IOL in her eye, and that afternoon she was thrilled to see clearly, not only the sign on a tall building in the distance, but also the thin second hand on her watch. 46 is the age when you need to start wearing presbyopia, but she said goodbye to presbyopia for good. How did this happen? The design of multifocal IOL, simply understood, is to make presbyopia directly on the IOL, so as to realize the function of having full vision without wearing presbyopia. Since the introduction of the new multifocal IOLs in the Chinese market, we have been able to free many patients from the trouble of presbyopia. I still remember a couple of university professors who traveled all over the world after retirement, they told me with great pride that their groupmates always had to look for presbyopia glasses to read the newspaper, but they were able to pick up the newspaper and read it directly, which made them envious of other people. Young people’s eyes are like a fine camera with stepless focusing function. This focusing function is realized by the deformation of the intraocular zoom lens —- lens. With age, the lens gradually hardens and becomes cloudy, the deformation ability decreases, and the focusing function gradually decreases until it is completely lost and becomes presbyopia. Most of the IOLs do not have focusing function and are called monofocal IOLs. More than ten years ago, most of the patients who underwent cataract surgery and implanted monofocal IOLs were completely “presbyopic”. With the development of science and technology, in the past ten years, some famous companies have developed multifocal IOLs, which have been continuously improved, and have been able to help most of the patients to realize their wish of not wearing glasses after cataract surgery. Is it possible for all patients to get rid of the annoying and inseparable presbyopia through surgery? No. Although multifocal IOLs provide full vision, they are picky and can only function well after implantation in eyes with good fundus function and no other problems except cataracts.
