Hepatic biliary stasis in pregnancy is a complication specific to the middle and late stages of pregnancy, clinically characterized by pruritus and elevated bile acids. It mainly jeopardizes the fetus and increases morbidity and mortality in unborn children. Clinical manifestations: i. Pruritus. The first symptom in almost all patients is itching without skin damage occurring in late pregnancy, which occurs after 30 weeks in about 80% of patients, and even earlier in some cases. The degree of itching may vary, often persistent, light during the day and heavy at night. Itching usually starts from the palms of the hands and feet, and then gradually to the proximal extension of the limbs, and can even develop to the face, but rarely invade the mucous membranes, itching symptoms for an average of about 3 weeks, sometimes up to several months in labor and delivery of a few hours or a few days to relieve the disappearance. Second, other symptoms, severe itching can cause insomnia, fatigue, nausea, vomiting, loss of appetite. Third, physical signs. Self-skin may have scratches, 10%-15% of patients in itching days or weeks of mild jaundice, some cases of jaundice and itching at the same time, in the delivery of a few days to subside, may be accompanied by deepening of the color of the urine and other manifestations of hyperbilirubinemia. There are no signs of acute or chronic liver disease in pregnant women, and the liver is large but soft.
