Proliferative scarring can be classified into three categories such as immature or red proliferative scarring for symptomatic treatment, with different protocols, such as silicone preparations for immature ones. The three common categories of proliferative scarring include immature or red proliferative scarring, linear proliferative scarring due to surgery or trauma, and proliferative scarring due to burns. 1. Immature or red proliferative scarring: Usually, onion extract-containing preparations, hypoallergenic paper tapes, and silicone preparations are used as prophylactic treatments. If the redness continues, linear proliferative scarring can be treated, or laser treatments (including fractional lasers and pulsed-dye lasers) can be performed. 2. Linear proliferative scar caused by surgery or trauma: Silicone preparation, fractional laser and pulsed dye laser are preferred in the proliferative stage, among which fractional laser can be used in the mature stage of linear proliferative scar, and exfoliative fractional laser is better than non-exfoliative one. If silicone preparations are ineffective or if the condition is severe and accompanied by itching, topical glucocorticoid injections, etc., may be used. Linear hyperplastic scar can be taken to surgical excision, surgical loosening, skin grafting and other programs if necessary. 3. Post-burn proliferative scarring: professional care is needed first, and scar prevention and treatment is carried out after the trauma is stabilized. Silicone preparations are preferred, and can be combined with the application of onion extract preparations, compression therapy, laser therapy, massage and physiotherapy, and so on. It is recommended that patients who are not feeling well go to the regular hospital in time, and follow the doctor’s instructions for symptomatic treatment.
