Pioglitazone has blood sugar lowering properties and is used for glycemic control in type 2 diabetes. Pioglitazone belongs to the class of thiazolidinediones, which is an oral drug that lowers blood glucose by increasing peripheral and hepatic insulin sensitivity. It is used clinically for the treatment of type 2 diabetes mellitus in which exercise and diet are not effective in controlling blood glucose. Adverse effects include heart failure, edema, elevated liver enzymes, and hypoglycemia. The drug is contraindicated in patients with heart failure or a history of heart failure, severe hepatic or renal dysfunction, comorbid bladder cancer disease, the presence of unexplained hematuria of the naked eye, drug allergy, type 1 diabetes mellitus, diabetic ketoacidosis. Pioglitazone needs to be used under the supervision of a clinician.
