An elderly female patient with “severe coronary artery disease, diabetes mellitus and old infarction” was admitted to the hospital. She underwent PCI stent implantation 3 months ago due to diffuse and severe coronary artery disease. After careful preoperative preparation, the patient was successfully operated by Prof. Liu Luqi a week ago with non-stop, original middle anterior descending branch stent debridement, expanded endothelial debridement coronary angioplasty and coronary artery bypass grafting, which was a difficult and risky operation that took 5 hours and was successful. Restenosis after stenting is a common complication, and bypass surgery is often considered the only option as a remedial measure, but in this case, the operation is prone to complications such as thrombosis, and the operation is very risky and technically demanding, so many specialized surgeons are discouraged. In this patient, the long segment (4 cm) of the anterior descending branch was stripped of its lining, the stent was removed at the same time, the vessel was repaired and shaped, and the bypass was completed, and then the posterior descending branch was stripped and bypassed. The surgery was very difficult and marked that the level of bypass in the Department of Cardiac Surgery of our hospital reached the advanced level in China. Cardiac surgery correspondent: Lu Ping
