What are the causes of sinus stenosis?

  Stenosis of the gastric sinus can occur in many intragastric and extragastric diseases, such as invasive carcinoma of the gastric sinus, chronic gastric ulcer, chemical burns of the gastric sinus, gastric sinusitis, gastric tuberculosis, and adhesions around the gastric sinus. Among the organic stenosis of the gastric sinus, cancerous stenosis is reported to account for 34.5%, chronic gastric ulcer and sinusitis for 61.2%, while among benign lesions gastric ulcer and gastric ulcer with sinusitis are the most common, accounting for 87.8%; simple sinusitis is less common, accounting for 9.7%.  The examination of gastrointestinal diseases mainly uses barium contrast, whose value is comparable to that of endoscopy. usg and ct are of special significance for understanding the internal structure of gastrointestinal tumors, the degree of infiltration of the gastrointestinal wall and metastasis. The degree of deformation can be severe, but the mucosal pattern is still normal, and the two are not proportional, and the angle of intersection between the indentation or filling defect and the gastric wall is mostly obtuse. In contrast, those due to lesions in the stomach itself are more centripetal, with a more fixed morphology, preceded by mucosal changes and consistent with the extent of stenosis in the sinus, with a smaller, acute angle of intersection between the filling defect and the gastric wall. The differentiation of benign and malignant stenoses is then considered. In general, the extent of malignant stenosis is more limited. It basically corresponds to the degree of stenosis. The mucosa is destroyed and lost, with irregular polyp-like filling defects; the gastric wall is stiff and fixed in shape; the entrance to the stenotic segment is large and flared or funnel-shaped; the segment above the stenosis is dilated and clearly demarcated, and there may be a “shoulder sign” or “sleeve sign”. The base of the duodenal bulb may show asymmetric indentation. In contrast, benign stenosis is mostly centripetal and relatively extensive, with thickened mucosa and occasionally smooth nodular translucent areas without mucosal destruction, with normal contraction but poor dilation; the entrance is small, and the proximal end may have a “round shoulder” or “straight line” shape. The base of the duodenal bulb occasionally has pressure marks, but both sides are symmetrical, and there may be prolapse of the gastric mucosa.  USG and CT are of special significance for understanding the internal structure of gastrointestinal tumors, the degree of infiltration and metastasis of the gastrointestinal wall. The combined application of these methods can provide a strong basis for the classification of the stage of gastrointestinal tumors and the determination of the treatment plan.
