Stress incontinence is especially common in middle-aged and older women, especially in postmenopausal women. According to surveys, the number of people suffering from urinary incontinence is even much higher than the number of people suffering from osteoporosis, diabetes and Alzheimer’s disease. A high percentage of patients believe that urinary urgency, frequency, nocturia and urge incontinence are natural symptoms of old age and tend to tolerate them, thus delaying treatment. There are two main types of treatment for urinary incontinence: surgical and non-surgical. The non-surgical treatment is suitable for patients with mild to moderate urinary incontinence and includes: medication, pelvic floor muscle exercises, biofeedback methods, electrical stimulation, acupuncture, and various aids. The main mechanism is to enhance the tone of the pelvic floor muscles and urethral muscles through aids, thus improving the function of the urethral sphincter. For patients with moderate to severe stress urinary incontinence, surgical treatment is often required – TVT/TOT minimally invasive sling procedure, which uses a special puncture needle to puncture through a small incision in the anterior vaginal wall and on both sides of the urethra, and a polypropylene mesh band is suspended in a U-shape below the middle part of the urethra, adjusting the position of the mesh band in a tension-free state to control the urine from overflowing. The surgically inserted mesh band together with the surrounding hyperplastic tissue forms a “hammock-like” structure that replaces the flaccid, extended pelvic floor support structure and the pubic urethral ligament, thus restoring normal urethral closure in patients with urinary incontinence, and the self-adhesive nature of the mesh band makes it self-fixing without sutures. This minimally invasive procedure has become the “gold standard procedure” for the treatment of female urinary incontinence because of its quick recovery, short hospital stay of 2-3 days, few safety complications and an efficiency of more than 95%.
