Which specialty therapies are commonly used for psoriasis?

As the name implies, “winter diseases” refers to certain diseases that tend to develop in winter or worsen in winter, such as chronic bronchitis, bronchial asthma, allergic rhinitis, rheumatic and rheumatoid arthritis, gastric and duodenal ulcers, coronary heart disease, frostbite, etc. “Summer treatment” refers to a unique treatment method in Chinese medicine to prevent the recurrence of old diseases or to reduce symptoms by taking advantage of the remission season in summer, when the Yang energy is strong. In recent years, summer treatment of winter diseases has become popular throughout the country, and this treatment has been carried out in hospitals and clinics of all sizes, and is favored by doctors and patients, and is still flourishing. The author has been engaged in this therapy for more than 30 years, and has gained some insights, so I will not speculate on the absurdity of the omission, and I would like to present my views on the errors and mistakes, and hope that my fellow teachers will enlighten me.
The therapy is based on the principle of “Heaven and man correspond to each other” in Chinese medicine and “treating the symptoms at the time of hair, treating the root cause at ordinary times” in disease treatment, and the method of prevention and treatment of disease. It includes internal and external treatment, drug and non-drug therapy. At present, most people think that the compressing method of three volt days is the winter disease summer treatment, which is too limited. Compressing is only one kind of drug external treatment method in winter disease summer treatment.
Other therapies such as internal herbal tonics, pills, pans, creams, pills and baths, acupuncture, tui na, etc. are also treatments for winter and summer illnesses. Why do we treat winter diseases in summer? Su Wen – Four Qi Tuning Shen Da Lun said: “The yin and yang of the four seasons, the root of all things, so the sage spring and summer nourish the yang, autumn and winter nourish the yin, in order to from its root”, and also said: “the sage does not treat the sick to treat the sick”. Winter illnesses are treated in summer, when the body’s Yang energy is at its peak, according to the above theory of “nourishing Yang in spring and summer” and the preventive method of “treating the untreated diseases”. In winter, there will be less or no illnesses, or the symptoms will be mild. Because winter illnesses are mostly due to weakness of the righteousness and the feeling of cold, winter is the main gas of cold, so it is easy to develop or aggravate in winter. In summer, with the help of the Yang heat of the sky, and warming drugs (non-drug therapy can also be applied in summer to warm the Yang and increase the effectiveness), so that the patient’s Yang energy is full, the positive energy is strong, the body contains the ability to resist the cold, and in winter, the positive energy is sufficient and the cold evil go, to achieve the purpose of prevention before the disease.
In 1972, Professor Tian Congwen of the Chinese Academy of Traditional Chinese Medicine was the first to treat asthma and chronic bronchitis with the application of compress therapy on the three volt days, and over the past 40 years, the treatment methods and types of diseases have gradually increased. In addition to the aforementioned compressing therapy, there are many drug and non-drug external treatments such as drug fumigation, foot bath, external application, navel patching, acupuncture and massage, cupping, gua sha and so on. At the same time, the diseases treated are gradually expanding, such as allergic rhinitis, pneumonia in children, recurrent upper respiratory tract infections in children, easy to catch a cold, peptic ulcer, chronic colitis, rheumatoid arthritis, cervical and lumbar spine lesions, frostbite, etc. However, winter diseases should be viewed objectively. However, it should be viewed objectively that there is a certain range of winter diseases and summer treatment, which cannot cure all diseases.
Notes on winter and summer treatment. Winter and summer treatment should be based on evidence-based treatment, according to the specific disease and evidence, combined with the patient’s physique, age, disease duration and other comprehensive analysis, respectively, the use of different treatment methods. Take the acupuncture point patching as an example, and introduce its precautions. First, selected herbs are crushed into fine powder, mixed with ginger juice, and applied to specific acupuncture points and fixed. Secondly, the duration of application is generally half an hour to one hour for children and one to three hours for adults. The duration of application can be decided according to the local sensation after the application. If the paste after the local comfortable slightly itchy, can be appropriate to extend the paste time, but not to burn the skin is appropriate; if the paste after the local hot feeling obvious, or even burning pain, the skin { red, can immediately go to the drug to prevent burns on the skin.
If the skin burns blistering, available glycolic lotion external application (where the patient with blistering, the effect is very good). Thirdly, during the period of patching, you should stay away from air conditioning and eat less cold and frozen food from cold drinkers to prevent the cold from stagnating in the body and affecting the therapeutic effect. Also should avoid eating spicy and fatty food, to prevent its dampness evil into heat and hurt Yin. Fourth, emotional optimism, establish confidence in curing the disease, maintain adequate sleep, moderate physical exercise to enhance physical fitness, it will help the consolidation of the effect of winter and summer treatment.
  1.Sealing treatment
After the patient’s skin lesions with compound zinc oxide paste or other drugs coated externally, with plastic film according to the size of the lesions to expand 2 ~ 75px ruling or cut large and small square, such as the lesion area is too large, two films can be spaced between 2 ~ 75px, and the needle in the film to tie the hole, in order to facilitate the local ventilation of the skin and reduce discomfort; we usually also often gauze bandage package, sealing time for 4 hours, once a day; winter after the application of drugs The sealing time is 6~8 hours, once a day (better at night), 7 days for a course of treatment. If the lesions are particularly thick, salicylic acid ointment can also be used to seal the package, the same method as before.
The method of sealing can make the drug persist in the skin lesion, maintain the effective concentration of the drug, continuous absorption, enhance the penetration of the drug, soften the stratum corneum, accelerate the scaling off, and promote the recovery of the skin lesion as soon as possible.
  2.Fumigation method
It is also called steam fumigation method and Chinese medicine steam bath, which is a method to prevent and treat diseases by fumigating the affected area or the whole body with the medicinal gas evaporated by heating the drug decoction. Fumigation method has two kinds of whole body fumigation and local fumigation, the drug used will be heated and boiled, evaporated gas, the patient naked whole body or partial sitting in the fumigator, so that his head exposed, the instrument temperature gradually increased to 40 ° ~ 50 °, the time is 20 ~ 35min, daily or every other day, for more lesions or diffuse the whole body, the general hospital more use this method.
  3.Fumigation method
Fumigation method is to decoct the drug, pour the soup into the bath or bath tub, place the affected area on the fixed apparatus while it is hot and fumigate with hot air, when the temperature of the liquid is suitable, drench and dip the whole body or the affected area with the liquid, the time is 20~30min, the wash is generally suitable for 40°~45°, too hot can cause burns, too cool is not effective enough, mainly for psoriasis with thicker skin lesions and more limited area, this method This method can be applied to both hospitals and families. Wipe dry the fumigation solution after about 20 min each time, pay attention to avoiding wind and keeping warm during the period, once/day, and apply emollient after drying the fumigation solution, you can use licorice oil for external application.
Chinese herbal fumigation treatment prescription: Phellodendron Bark 30g, Mugwort 30g, Pepper 30g, Phellodendron Bark 30g, Ground Bone Bark 30g, Bai Xian Pi 30g. Decoct about 4L, dilute with warm water 1:10 and pour into a wooden bath.
  4.Sweat method
Sweating method, also known as wet compress method, is the use of decoction while hot wet gauze 6 ~ 8 layers or towels of considerable thickness, such as dressings, wringing and squeezing to not drip for the degree of the affected area for a few minutes, to dry the dressing and then dipped in the liquid, sweating the affected area, each time the compress time of about 20 ~ 30min, several times a day. Wet dressing because the liquid evaporates, you need to prepare two copies of the dressing alternately, or the liquid frequently drip on the dressing to ensure that the dressing appropriate temperature and humidity, mainly for the extremities or head and other small skin lesions patients, the family applicable.
  5.Bathing method
Bathing method is a method to treat diseases by bathing with decoction of medicine. Decoction of selected drugs, and then add the liquid to the hot water in the bath or bathtub, while hot dip local or whole body. This method is suitable for those with large skin lesions and is applicable in home hospitals.
You can use the formula: beehive, g sign grass, ground bark, raw earth, through the bone grass each 50g, bitter ginseng, white fresh skin each 40g, Dan Shen, snake bed seed each 30g, safflower 20g. 1 dose of the above herbs, decocted and added to the wooden bath, add water about 100L, adjust the water temperature to 38 ° ~ 42 °, the patient naked into the bath soak, each soak 20min, once a day, continuous bath for 3 weeks.
  6.Scouring method
Scrubbing method is a treatment method of decocting medicine and scrubbing the affected area with gauze block. The selected drug will be decocted with water, remove the slag, and scrub the affected area with gauze when the juice is warm, preferably until the scales fall off and the skin is slightly red, mostly used for those with thicker skin flakes, suitable for the patient’s own operation to scrub the extremities and chest, waist and abdominal area, and is used by families.
  7.Compounding the navel
Patients clean the navel, then put the appropriate medicine into the navel, and fix it with gauze or medical tape, and replace it with new medicine after 24 hours.
  8.Cutting ear therapy
After routine skin disinfection, pull the back of the ear flat with the left hand, put the middle finger on top of the bottom, hold a sterilized needle in the right hand, cut an incision about 0.5-1cm long, bleed about 0.5mL, wait for natural hemostasis, and if the bleeding is more than that, press to stop bleeding. After 2 days, repeat the treatment in the same way as above, 7 times a course. Rest for 2 days during the course of treatment, and carry out two courses of treatment in total.
Method 2: The patient sits on a stool with the whole body relaxed, and the doctor rubs the back of the ear on one side of the patient with the thumb and finger with moderate strength up and down repeatedly until it is red and hot and the local superficial vein at the heart point is seen to be turgid, and after routine disinfection, the doctor pinches the earlobe with the thumb and index finger of the left hand and pulls the back of the ear downward to flatten it, and the middle finger tops it from the earnail boat to the back of the ear, and the right hand holds a small sterile scalpel and follows the turgid superficial vein longitudinally with the tip of the knife. Accurately, quickly and gently make a small incision of about 0.5 cm (in order to cut the outer wall of the superficial vein without damaging the ear cartilage), and after the incision, wipe the incision repeatedly with a cotton ball dipped in alcohol so that the blood flows out naturally. The procedure is performed twice a week, alternating between the left and right ears, 15 times for a course of treatment. Rest for 10 days before proceeding to the next course of treatment. It is important to pay attention to strict aseptic operation to prevent secondary infections and to control the technique of cutting and treatment to prevent improperly cutting the cartilage of the ear. The patient should have a good explanation before cutting and treatment to prevent dizziness.
Method 3: Disinfect both ears with alcohol, cut the vein along the upper dorsal vein from the ear wheel to the root of the ear with a small scalpel in sequence (Figure 1), do not hurt the cartilage membrane, let the natural bleeding 0.2~0.5 ml, wipe off the blood drops, apply a little alkaline weathering to the incision, and use sterile dry cotton balls to stop bleeding for 5 or 10 minutes. Once every 5 days, six times for a course of treatment. Avoid eating spicy food and drinking alcohol during the treatment period, and do not cooperate with internal and external medicine.
  9.Blood pricking therapy
  Methods are as follows.
  (1) Selection of acupuncture points: select 3~4 unilateral more serious skin lesions; also select contralateral lung Yu, diaphragm Yu blood sea; add Quchi for heavy upper limb skin lesions, Feng Shi and Yin Ling Quan for heavy lower limb skin lesions, add Wei Zhong for heavy lumbosacral skin lesions, and release blood from the tip of the ear for heavy head lesions. Take no more than 10 points at a time.
       (2) Operation: After routine local disinfection, use a blood collection needle to puncture the lesion vertically for 3 to 5 stitches at a depth of about 2~6mm, and then remove the needle quickly, followed by rapid cupping of the fire pot on the blood-pricked area.
The can is left in place for 3~5min, bleeding 3~10mL per can, and the can is opened after the blood has basically coagulated; for ear tip bleeding, a blood collection needle is used to puncture and squeeze 2~3 drops of blood. The blood is routinely disinfected and no special treatment is necessary.
All the above methods can be combined with ultraviolet radiation together, which can improve the efficacy. NB-UVB treatment is given, the specific operation is to use the whole chamber NB-UVB treatment instrument, the wavelength is 311nm, the patient wears special glasses, wears triangle underwear to protect the perineum, when irradiating the patient to receive treatment parts placed in the machine radiation center point 21cm, choose the initial irradiation dose 0.3J/cm2, later according to the patient’s skin reaction degree to adjust the irradiation dose, generally each time according to The initial irradiation dose is 0.3 J/cm2, and the irradiation dose is adjusted according to the patient’s skin reaction, generally in increments of 0.1 J/cm2 each time. If the irradiated area appears obvious erythema, then do not increase the dose for the time being; if the irradiated area appears heavy erythema or blister, then stop the treatment, and after the lesion basically subsides, no more increment. Irradiation every other day.
The lesions need moisturizing care, you can apply licorice oil or other emollient cream.
In addition, there is the application method, which is a cream prepared according to the different conditions and applied externally to the affected area.
