To investigate a clinical staging of adult degenerative lumbar scoliosis that is useful for guiding surgical treatment. [Type I: minimal or no rotation; Type II: significant rotation and displacement; Type III: severe rotation and displacement with coronal and/or sagittal trunk imbalance (C7 plumbline shift > 4 cm). Type III: severe rotation and displacement with coronal and/or sagittal trunk imbalance (C7 plumbline shift > 4 cm). Type A: low back pain without radicular pain; Type B: radicular pain from lumbosacral compensatory scoliosis with or without low back pain; Type C: radicular pain from voluntary scoliosis with or without low back pain. [Results] There were 3 cases of type ⅠA, 28 cases of type ⅠB, and 6 cases of type ⅠC; 21 cases of type ⅡA, 45 cases of type ⅡB, and 39 cases of type ⅡC; 12 cases of type ⅢA, 6 cases of type ⅢB, and 8 cases of type ⅢC. Among them, posterior posterolateral intervertebral fusion or transforaminal intervertebral fusion was the main method for type ⅠA; simple spinal canal decompression was the main surgical method for type ⅠB and ⅠC; posterior canal decompression plus posterior posterolateral fusion was the main method for type ⅡA, ⅡB and ⅡC; anterior intervertebral fusion plus posterior instrumentation orthopedics, spinal canal decompression and posterior posterolateral fusion was the main method for type ⅢA, ⅢB and ⅢC.
