Treatment of osteoarthritis of the patellofemoral joint

  Patellofemoral joint osteoarthritis is one of the main causes of knee pain, and it has been reported that the incidence of simple patellofemoral arthritis was found to be 9.2% in an analytical study of radiographic examinations of people aged over 40 years who had anterior knee pain, while its incidence reached 6.3% in the middle-aged and elderly population in China.  Due to the long-term wear and tear of the patellofemoral joint, increased intraosseous pressure leads to a series of degenerative changes such as softening and proliferation of articular cartilage, whose pathology is characterized by degeneration of the protofibers of patellar cartilage, scaly fragmentation, and finally cartilage erosion exposing the bone. Early diagnosis and treatment are important to slow down its further development. At present, in addition to the short-term use of anti-inflammatory and pain-relieving drugs for symptomatic relief of symptoms, oral medication for this disease in Western medicine is more often used in the long-term treatment of aminoglycosides to protect the articular cartilage, slow down its degeneration and anti-inflammatory and pain-relieving effects to delay the development of knee osteoarthritis.  Osteoarthritis of the patellofemoral joint is equivalent to “paralysis”, “strain”, “injury to tendons” and other diseases (evidence) in Chinese medicine. According to the theory of injury treatment by Zong Shih’s injury department, “the treatment of both evil and phlegm and dampness is the only thing that matters; the tendons and bones are both important, and the liver and kidneys are combined”, the occurrence of this disease is mostly related to phlegm, stasis and deficiency (deficiency of the liver, spleen and kidneys). The deficiency of the liver, spleen and kidney is the main cause, and the interplay of phlegm and stasis is the symptom, which is the evidence of deficiency. “The kidney is the master of bone and marrow, and the liver is the master of tendons and blood collection. Spleen is the essence of the latter”, deficiency of the spleen and kidney, the role of kidney Yang warming is reduced, the spleen is not healthy, internal dampness is generated, the spleen’s transportation and transformation is not in charge, the innate essence is not supplemented by the source, aggravating the loss of kidney essence, water and dampness within the stagnation, for a long time will be gathered into phlegm, run through the meridians, blocking the qi flow; “Ling Shu” has a cloud: “blood and then the meridians are popular, Ying restores Yin and Yang, tendons and bones are strong, joints Clearly, the joints are clear”. The combination of phlegm and dampness, together with wind, cold and dampness, causes stagnation of the bones and veins, resulting in stagnation of phlegm and stagnation of the joints, and pain if the joints do not pass. The cartilage ischemia, damage, edema, and increased intraosseous pressure seen in this disease are actually due to deficiency of the liver, spleen, and kidneys, and phlegm and stasis blocking the meridians and bones. Therefore, the treatment of this disease focuses on both the symptoms and the root cause, and the liver, spleen, and kidney are tonified together while harmonizing the phlegm, so that the positive energy is sufficient, the phlegm and stasis are eliminated, and the tendons and bones are strengthened.
