What is synovial sarcoma Synovial sarcoma is a highly malignant soft tissue tumor originating from synovial membrane, bursa and tendon sheath. It is rare, more frequent in adolescents and young adults, and often occurs in the knee and thigh, foot and ankle, shoulder, arm, elbow, wrist and hip, manifesting as a local mass, mostly painless and non-itchy, so it is mostly found in advanced stage. Lung metastases are the most frequent. Surgery is the standard modality for the initial treatment of the disease. Its survival rate is related to the size of the tumor. When the diameter is <5cm, the five-year survival rate is 88%, while the five-year survival rate is 38% and 0% for 5cm~10cm and >10cm diameter, respectively. The 10-year survival rate for primary was 55%, while the 10-year survival rates for recurrence and metastasis were 11% and 15%, respectively. Currently, there is no standardized and unified protocol for the treatment of synovial sarcoma, and the core issue remains how to improve and stabilize the survival rate, reduce the recurrence rate, and improve the quality of life of patients, so comprehensive treatment is advocated. Recently, there has been a lot of interest in synovial sarcoma or sarcoma as a type of disease. Sarcoma, like cancer, is a malignant solid tumor. Surgery is the main tool, radiotherapy has some efficacy, and it is also a more difficult disease to treat among tumors. Chinese medicine has some advantages in preventing recurrence and metastasis after surgery. Spleen deficiency and dampness obstruction is the root cause Western medicine is not clear about the causative factors of synovial sarcoma, so it is difficult to develop preventive measures. I have treated many patients with sarcoma in clinic and found that most patients with synovial sarcoma have weak spleen and stomach, prefer sweet or cold food or meat, and are impatient and irritable. Therefore, in clinical evidence, they are mostly identified as spleen deficiency, dampness, blood stasis and cancer toxicity. The reasons are as follows: 1. Synovial membrane is the inner layer of joint capsule, rich in blood vessels and secreting synovial fluid; intraoperative patients with synovial sarcoma can be seen to secrete mucus and gelatinous fluid; prone to lymph node metastasis, mucus and malignant lymph node metastasis are mostly identified as phlegm and dampness in TCM. 2, easy to recur after surgery, which is related to the dampness is sticky and difficult to get rid of; and dampness belongs to yin evil, which is good for radiotherapy (identified as heat toxicity in Chinese medicine). 3, Synovial sarcoma mostly occurs in the extremities, which are dominated by the spleen; and synovial sarcoma occurs in the muscle layer instead of bone, which is also dominated by the spleen, so synovial sarcoma is mostly seen in spleen deficiency. 4, easy to lung metastasis, that is, related to spleen deficiency, also related to blood stasis, spleen deficiency and lack of qi, qi and blood stagnation affect the lung loss of catharsis, easy to appear lung metastasis. In our opinion, TCM treatment of synovial sarcoma adopts tonifying the spleen, dispelling dampness and activating blood to fight cancer. The formula uses Liu Wei Di Huang Wan, Dou Hua Zai Sheng Tang and Body Pain and Stasis Removal Tang with addition and subtraction, and the anti-cancer drugs are mostly Gecko, Zanthoxylum (note the toxicity), Money White Flower Snake and Toad Skin. Gecko and toad skin have better effect on lymphatic metastasis. Lymph node metastasis should be treated according to different metastasis sites. If lung metastasis alone occurs, Blood Mansions and Stasis Removal Soup can be added to the treatment of lung tumors, which is effective in some patients. Acupuncture plays an important role in preventing recurrence and metastasis For synovial sarcoma, surgery is the main treatment. The advantage of Chinese medicine (Chinese medicine + acupuncture) in the treatment of synovial sarcoma is in preventing its recurrence and metastasis. It is important to emphasize that acupuncture plays an important role in preventing the recurrence of synovial sarcoma, because Chinese medicine emphasizes that “if positive qi exists inside, evil cannot dry up”. This will make the evil Qi less likely to invade and the tumor less likely to recur. Chinese medicine can also be used in conjunction with radiotherapy for synovial sarcoma to reduce its side effects. In my experience, if the primary tumor is larger than 2 cm, it is difficult for herbal medicine to make it disappear, so if surgery is possible, we will strive for surgery again. For this disease, we suggest also suggest combined Chinese and Western medicine treatment. It must be emphasized that no tumor can be solved by any single means, but must be treated in an integrated way, including surgery, radiotherapy, chemotherapy, and Chinese medicine.
