What to do about breast lumps

  Breast lumps are common clinical diseases in mammography, which can occur in both men and women, and are most common in women. Breast lumps can be formed due to trauma, inflammation, benign malignant tumors, etc. Malignant breast lumps account for about 20-25% of women’s breast lumps and are the second most malignant tumor in women, second only to cervical cancer. Therefore, we should be more alert to breast lumps in women, and careful examination is the key to improve the effectiveness of breast lump diagnosis and treatment to prevent missed diagnosis or misdiagnosis.   With the improvement of people’s living standard and the enhancement of self-care awareness, many women will do self-examination of their breasts or find breast lumps during physical examination, for which some people will be worried and nervous all day long, worrying that they are suffering from breast cancer; while some people are the opposite, I have a patient who originally had a small lump of 1cm in size, but let it go, and two years later it was already advanced breast cancer, which resulted in delaying the disease. So when we find a breast lump, how should we handle it correctly? Han Jing, Breast Surgery Department, Shanghai Oriental Hospital First of all, we should know that there are many causes of breast lumps, among which there are benign, malignant and junctional. Benign ones include: fibroadenoma, cyst, lipoma, mastopathy mass, intraductal papilloma, inflammatory mass, etc.; malignant ones include primary and secondary breast cancer; junctional ones are mainly lobulated tumors.  In fact, the best way to find a breast lump is to go to the hospital for examination in time. Through the doctor’s examination and some auxiliary examinations, such as breast ultrasound, mammography and MRI, the nature of breast lumps can generally be judged, and the doctor will also make corresponding treatment and therapy advice according to different lumps.  The current disposition plan for breast lumps is mostly based on the BI-RADS rule.  The BI-RADS is the “Breast Imaging Reporting and Data System” recommended by the American College of Radiology, which is a more standardized reporting system. The significance of each category is as follows: Category 0: Needs to be recalled and evaluated in conjunction with other examinations. This indicates that the information obtained from the examination may not be complete.  Category I: No abnormality is seen.  Category II: benign changes are considered and regular follow-up is recommended (e.g., once a year).  Class III: Benign disease is possible but requires a shorter follow-up period (e.g., once every 3 to 6 months). The percentage of malignancy in this class is less than 2%.  Class IV: There are abnormalities that do not completely exclude the possibility of malignant lesions and require biopsy for clarification.  Class IVa: low likelihood of predisposition to malignancy.  Class IVb: medium likelihood of predisposition to malignancy.  Category IVc: high probability of malignancy.  Category V: High suspicion of malignant lesion (almost identified as malignant disease), requiring surgical excisional biopsy.  Class VI: Malignant lesion has been confirmed by pathology.  Of course, the above is only a judgment of the degree of breast disease from imaging, the specific situation should be combined with the clinician’s judgment for a comprehensive analysis, depending on each individual case.  In our breast surgery practice, we take a proactive approach to the management of breast lumps, and we need to achieve the “three early stages” of early detection, early diagnosis and early treatment. Even for BI-RADS II and III lumps, as long as the lumps are indeed detected, early biopsy is often taken to provide early diagnosis and early treatment to relieve psychological tension and restore normal life.  Early detection and treatment of breast lumps is very important, in ordinary life, you can self-examination or regular hospital breast examination, early detection of breast lumps; in addition, usually strengthen exercise, happy mood, regular life, light diet, high fiber and low fat, breastfeeding, etc., can prevent the occurrence of breast lumps.
