There are various manifestations of big toe exostosis, protrusion of the big toe to the medial aspect of the joint between the big toe and the ball of the foot and bunion. The protruding area can be painful to wear shoes. If the extrusion is prolonged, the bunion squeezes the second toe appearing as a painful extruded area between the bunion and the second toe, making it impossible to wear shoes. If further aggravation occurs, the second toe rides across, the second toe is elevated, the big toe rotates in front, and pain occurs in the position of the big toe, the dorsal side of the second toe, and the palm of the foot, forming a deviated toe. As a result, when big toe ectropion occurs, the patient may experience pain on the bottom of the foot and forefoot with or without shoes, which has a significant impact on the patient’s life.
