Mammary gland hyperplasia with hyperplastic nodules

Patient: Female, 22 years old, two months ago she noticed breast pain and felt a lump in her left breast that moved. After the doctor’s visit, a new lump grew in the upper outer quadrant of the right breast (just after menstruation), with pain, and after each menstrual period, the lump became painful and grew. Ultrasound findings: Bilateral breast symmetry, left breast gland is about 1.2 cm thick, right breast gland is about 1.1 cm thick, irregularly arranged, echogenic thickening and enhancement. The left and right breast ducts were about 0.24 cm and 0.32 cm, respectively, with scattered foveal hypoechogenicity of different sizes and unclear boundaries. Hypoechoic nodules were seen in the left upper inner breast quadrant and right upper outer breast quadrant, approximately 1.5cmx1.1cm, 0.6×0.3cm, 0.8×0.4cm, with clear borders and uniform internal echogenicity. CDFI showed no abnormal color flow bilateral axillary non-circular hypoechoic mass ultrasound suggests: bilateral breast hyperplasia bilateral solid breast nodules, suggest follow-up review. The original lumps have not changed, but later the middle lumps will grow after menstruation and are painful. Should I have surgery, western medicine or Chinese herbal medicine? Qin Yuenong, Department of Breast Medicine, Shanghai Longhua Hospital: Your case should be breast hyperplasia with hyperplastic nodules or fibroadenoma formation. If the fibroadenoma is fully formed (it depends on whether the nodule shown by ultrasound has an envelope or not, with an envelope it is a fibroadenoma), it is difficult to be eliminated by medication. If only one of the three nodules is more than 1cm, you can start with medication and regular observation, and if the nodule grows up, you can have surgery in time, or continue to observe it. In medicine, there is no special medicine in western medicine, choose Chinese herbal medicine or proprietary Chinese medicine. Patient: Now the swelling in the upper quadrant of the right breast is growing again, there is tenderness, the breast is not painful, the lump will move, feel like a nodule from a section of the mammary gland suddenly rising, because it can only move from side to side, not up and down. I had to take the emergency contraceptive pill about 10 days ago because of the failure of birth control, is it related to this? The lump usually hurts for a few days after the period, but now it’s been more than 10 days since the period, why does it still hurt? Shanghai Longhua Hospital Breast Department Qin Yuetnong: Oral contraceptive pills are used to adjust the hormones in the body, which can indeed have a negative effect on breast enlargement. The lump growth and pain are probably related to this. Wait for a period of time the pain will be relieved. It is also recommended to review the ultrasound to observe the size of the lump on a later date (after 2 more menstrual cycles, 3 or 4 days after the end of menstruation).
