In years of clinical practice, we have seen too many breast cancer patients like Chen Xiaoxu, rural women, factory workers, dancers, university professors, acting stars …….. Although they have different social classes, different professional qualities, different education levels and different economic situations, they all have similar states of mind. Roughly speaking, they can be divided into the following categories: 1.Patients who have found out abnormalities on breast early, but due to the lack of knowledge about breast cancer, they think they cannot have cancer, so they just get over it and miss the best time for treatment; 2.Patients who have found out they may have breast cancer, but they are afraid of losing the stage of life and happy life, so they suffer from it and hide their ears; 3.Ignorant and ignorant This group of patients, who have been diagnosed with breast cancer, lack proper knowledge of scientific treatment methods, worry about the side effects of surgery, chemotherapy and radiotherapy, and refuse scientific treatment, and choose other witchcraft, the result is natural. In the 21st century, human beings have been able to say “no” to cancer, among which breast cancer can be considered the most researched, most clearly understood, most standardized, most abundant and most curable of all malignant tumors, and has been regarded as the model of cancer treatment in the industry. With more than 100 years of clinical research on breast cancer, especially the emergence of systemic therapeutic drugs in the past half century, and the comprehensive treatment of surgery, radiotherapy, chemotherapy, endocrine therapy and molecular targeted therapy, breast cancer has been able to obtain a high clinical cure rate and has formed a scientific standard diagnosis and treatment specification. Let’s take a look at how to prevent breast cancer, how to detect breast cancer in early stage, how to cure early stage breast cancer and how to treat advanced breast cancer. Surveillance of high-risk groups At present, it has been found that breast cancer incidence rate of breast cancer patients, those with family history of breast cancer, those with mutation of oncogene BRCA1/BRCA2, those who have never been pregnant or childless in their life or those who have their first child older than 30 years old are significantly higher than the average population. A series of targeted preventive measures can be taken for this group of high-risk individuals. For breast cancer patients, comprehensive checkups are generally performed once every 3-6 months after surgery for 2-3 years, and then every 6 months thereafter, and at least once a year after 5 years for follow-up. For women with a family history of breast cancer, a comprehensive checkup at the hospital should be done once a year after the age of 30. For patients with abnormal BRCA1/BRCA2 gene, 80% of them will develop breast cancer, so many people in the UK and the US have tried to have preventive double mastectomy and breast reconstruction treatment, which significantly reduces the possibility of breast cancer. The results of breast cancer prevention studies have been published. The results of breast cancer prevention studies have also shown that both triamcinolone acetonide and ryanodine can reduce the risk of breast cancer by 50%. Early detection of breast cancer The current international community recommends three methods of breast cancer screening: mammography, physician breast examination, and breast self-examination to detect early breast cancer. For breast masses that are difficult to define, MRI is helpful to clarify the nature of the masses. In China, as ultrasound is more popular, it is also an important tool for experienced doctors to make preliminary diagnosis of breast cancer. The latest information shows that the increase of breast cancer cure rate in Europe and America in recent years is mainly attributed to the development of early screening, which enables more early breast cancer patients to be detected and treated adequately. Curing Early Stage Breast Cancer Once a patient with early stage breast cancer is diagnosed, doctors can design a reasonable treatment plan for the patient based on the treatment protocols discussed by breast cancer experts worldwide. In order to avoid early stage breast cancer patients from taking a wrong turn, I think the following steps should be kept in mind: 1) Complete comprehensive systemic examination before surgery to clarify the clinical stage, pathological type, differentiation degree and molecular index status of breast cancer; 2) Carefully discuss whether there is any chance of breast conservation before surgery; 3) Develop scientific adjuvant chemotherapy, radiotherapy, endocrine therapy and molecular targeted therapy plans strictly according to the tumor risk classification after surgery. According to a large amount of data, for early stage I and II breast cancer patients, about 80% can be cured; and for locally advanced stage III patients, after neoadjuvant therapy, combined with other treatments, about 50% can be cured. From this, we can imagine that when patients of “pass-and-fail” type are careless, when patients of “worry-and-fail” type cover their ears, and when patients of “ignorant” type are imaginative, the chance of cure will be reduced from their hesitation and ignorance. The chance of cure also slipped away from their hesitation, uncertainty and fantasy. Although it is difficult to completely cure breast cancer with recurrence and metastasis, breast cancer patients still have a chance to survive with tumor for a long time through scientific treatment. The eight-word policy of “no change in formula, no change if ineffective” promoted by Professor Song Santai, an expert in breast cancer treatment, should be the principle for both doctors and patients to follow. At present, the systemic treatment of breast cancer includes chemotherapy, endocrine therapy and molecular targeted therapy, which can be applied alternately, just like our right and left hands, the left hand is replaced by the right hand when the left hand is tired, and the right hand is replaced by the left hand when the right hand is tired, and many of our patients have achieved long-term survival with tumor, and it is not uncommon to see patients with several years or even ten years. Therefore, even if breast cancer patients have recurrence and metastasis, it is never the end of the world, and you must not lose confidence. The Lin sister who fell from the sky is gone, taking away her ethereal and melancholy, leaving us an eternal and classical Lin Daiyu, and an inexhaustible benevolent love for the earthly world.
