Do not panic in a convulsive seizure

  I remember two years ago a media report in Wenzhou, a 4-year-old girl, summer night playing in Maanchi Park, because the fever is too high lead to convulsions, live in the vicinity of a kind old lady, specially from home to carry a large bottle of salad oil, a strong to the child’s mouth. The child was later taken to the intensive care unit of Yuying Children’s Hospital for resuscitation. After the doctor’s diagnosis, the febrile convulsions were not a big problem, but the aspiration pneumonia caused by filling salad oil became a potentially life-threatening problem. Fortunately, after the careful rescue and treatment of medical staff, finally saved the life, but the hospital stay lasted for more than a month, hospitalization costs up to 100,000 yuan huge.  It is no coincidence that in my long-term clinical work in pediatric neurology, I often see parents who bring their children to the outpatient clinic with salad oil. When asked why, they often reply with a strong voice, saying that there is a “toad” stuffed in the child’s throat when he or she has a cramp, and that it takes salad oil to make the “toad” run out. This is basically a theory of the older generation, and few people in the younger generation believe in it anymore.  However, when their own children encounter convulsions (cramps), is usually a six-headed, that scenario, you have to temporarily believe in the experience of the elderly, there are a variety of unscientific methods, useful spoon forcibly pry open the mouth, resulting in the incisors fall into the trachea, the need for emergency surgery to remove foreign bodies trachea; useful hand directly digging the child’s throat, resulting in laryngeal edema, the need for emergency tracheotomy; There are cases of mucosal burns caused by pouring boiling water directly into the mouth. These are all wrong methods, so when a convulsion occurs parents should how to properly deal with it?  When a child has a convulsion, parents should first be calm, do not panic, do not cry loudly, and do not shake the sick child, because this will aggravate the condition. First of all, the child should be quietly laid flat on the bed, head turned to the side, untie the collar, let its saliva and vomit as far as possible out of the mouth, and to remove, so as not to accidentally inhale into the lungs, to ensure that the respiratory tract is unobstructed. Then insert a cloth-covered chopstick or a thin spoon handle between the child’s upper and lower teeth to avoid biting the tongue, but there is no need to forcibly pry open the teeth that are already tightly closed. When the patient is convulsing, do not press his limbs hard to avoid fractures or sprains. You can fill some soft things (such as blankets, clothes, etc.) under his head and limbs to prevent his skin from abrasions.  For those who have a high fever and have a stroke, use a cool wet towel on the forehead of the sick child. If the spasms are strong, pinch the finger or prick the Renzhong point (in the upper 1/3 of the nasolabial groove) with a needle, which can usually stop the spasms temporarily. If you take the following treatment, the spasm can not be calmed down, so as to cause respiratory arrest, then immediately artificial respiration, and then immediately sent to the hospital, do not delay, adults should remember the time and performance of children’s convulsions, if the seizure has not stopped for more than 5 minutes or continuous seizures, should be promptly sent to the hospital for examination and treatment. After a convulsive seizure, there are 30 to 60 minutes of drowsiness, observe the child’s face, breathing and heartbeat are stable in all aspects, the child should be allowed to sleep and rest to facilitate the recovery of brain function.  Short-term convulsions are generally not life-threatening, but only convulsions that last more than 30 minutes and frequent attacks are life-threatening, and the chances of life-threatening events are increased when the scene is not handled properly.  Now that the weather is getting warmer, the number of febrile children is gradually increasing, and the chances of having convulsions are also increasing. Therefore, we would like to solemnly remind parents that when having a convulsive seizure, please do not: forcibly stuff the mouth with hard objects; take medication or drink water immediately; over panic; forcefully catch the patient; too many people around; argue with the patient.
