Surgery has three advantages: definite diagnosis, immediate relief of local symptoms, and stable control of brain condition. However, the decision of surgery should be based on 6 aspects: 1. Number of tumors: single tumor surgery is good, multiple tumors can achieve the same effect as single tumor if they are completely cut. 2.Size of tumor: tumor diameter greater than 3cm should be operated, less than 1cm is generally difficult to find tumor and patients have no clinical symptoms, generally consider radiotherapy. 1-3cm tumor is decided to be operated according to other factors. 3.Location of tumor: tumor located in brainstem, thalamus, basal ganglia, surgery is not recommended. 4.Neurological function status: KPS score ≥ 70 (KPS, Karnofsky Performance Scale) 5.Physical status, degree of control of primary lesion: able to survive for more than 3 months, no contraindication to anesthesia and surgery in all body systems. 6.Sensitivity of tumor pathology to radiotherapy: highly sensitive: lymphoma, germ cell tumor and small cell lung cancer. Moderately sensitive: breast cancer, non-small cell lung cancer and rectal cancer. Low sensitivity: melanoma, renal cancer and sarcoma. Insensitive focused on considering surgical opportunities.
