Some time ago, I had a fish hot pot, and I disliked the trouble of having to spit out the fish. The patient was a male, 50 years old, with no previous special medical history, and was admitted with stable vital signs and “chest pain for 6 hours” as the main complaint. In cardiovascular medicine, for patients with acute chest pain, the first step is to exclude the three major emergencies causing chest pain: 1) acute myocardial infarction; 2) aortic coarct; 3) pulmonary artery embolism; the patient’s pain symptoms had lasted for several hours, and the emergency electrocardiogram and cardiac enzymes did not indicate any obvious abnormalities; later, the patient was found to have an abnormal fine needle-like high-density shadow in the gastric cardia through the chest pain triple examination, and foreign body impaction was considered. When the patient was asked again about his past medical history, he remembered that he had accidentally swallowed a fish spike when he was eating fish yesterday, and he did not care about it because the foreign body sensation disappeared after swallowing rice. The patient was finally consulted by the Department of Otolaryngology, Gastroenterology and Thoracic Surgery, and the location of the fish spike was considered to be low and close to the large blood vessels, so he was finally referred to the Thoracic Surgery Department to remove the spike through open-heart surgery. In everyday life, people often try to swallow fish spikes by folklore methods: 1) swallowing solid food such as rice or steamed buns to try to swallow the spike; 2) drinking vinegar to soften the spike; however, such methods often do not solve the problem and may bring more hidden dangers. Swallowing rice, steamed buns and other food to swallow the fish spike is very dangerous because the spike may pierce the blood vessel, and drinking vinegar does not really soften the fish spike. Because vinegar can only stay briefly at the location where it is stuck, the effect is not only very limited, but may also delay the treatment. Depending on the location of the fish spike, it can cause different diseases; a fish spike stuck in the throat can cause a mediastinal infection that can damage large blood vessels such as the aorta, pierce the esophagus and lodge in the aorta of the heart, causing chronic bleeding in the aorta, cross-infection and aortic aneurysm, which can lead to life-threatening hemorrhage in severe cases. In severe cases, this can lead to life-threatening hemorrhage. A fish spike lodged in the perianal area can cause a perianal abscess. What should we do when we encounter a fish spike stuck in the throat? The right thing to do is to stay relaxed and not to try folklore methods when a foreign body is stuck in your throat. Do not swallow fish, chew slowly, and swallow all hard foods such as fish spines after removing them.
