Occult penis means that the penis is hidden under the skin, the penis has a short appearance, and the distance between the foreskin opening and the penis root is short. The dorsal side of the foreskin is short and the ventral side is long, with more inner plates and fewer outer plates. The foreskin wraps around the penis like a bird’s beak and is not attached to the body of the penis. If the skin around the penis is pushed back by hand, the normal penile body can be revealed. When a shallow groove is palpated on the dorsal side of the head of the penis during examination, it should be noted that it may be complicated by supraurethral cleft. Many occult penises are secondary to the accumulation of fat in the lower abdomen, especially the prepubic area, in obese children. If the prepuce opening is narrow, the clinical presentation is the same as that of the prepuce. Despite its resemblance to circumcision, occult penis is a completely different condition. Under normal circumstances, it is common for infants to have a small penis, which is a physiological phenomenon, but it will grow as they get older. If an infant grows to 5-6 years old and the penis is still not significantly enlarged, it only looks like a raised mound with a soft dermatome and an opening as thin as a pinhole at the front end, and although urination is still normal, the dermatome enlarges when urinating, this phenomenon is medically known as occult penis.
