Autologous reimplantation of severed penile wounds

The survival rate of severed penis replantation is higher because penile tissue is more resistant to ischemia compared to other organs. This may maintain survival; dissociated sites that are cryopreserved slow down the function of intracellular enzymes and reduce the cellular need for sugar, oxygen and nutrients, thus prolonging ischemic survival. wei reported successful reimplantation of penis with heat ischemia for 16h and cold ischemia for 24h. It is generally believed that the survival of reimplantation is impossible with thermal ischemia longer than 24h and cold ischemia longer than 72h. Penile reimplantation surgery, which should be performed by microsurgical techniques, can significantly improve the survival rate of reimplantation and restore the ability to have sexual intercourse. First of all, careful debridement should be performed to preserve as much viable tissue as possible, recognize the structures that need to be anastomosed at both ends, and suprapubic cystostomy. To stabilize the penis during reimplantation, a Foley’s ureter is first inserted from the external urethral opening. Redocking began with urethral anastomosis, followed by anastomosis of the cavernous artery with 10″0″ non-absorbable nylon thread; the white membrane was closed tightly with successive 4″0″ Dexon sutures; followed by anastomosis of the dorsal artery, vein, and dorsal nerve.
