Colorectal melanosis may cause gastrointestinal discomfort, and in a few cases, cardiac or generalized discomfort.
There are generally no specific signs or symptoms. Common clinical symptoms include gastrointestinal discomfort, such as bloating, constipation, and difficulty in defecation, and in a small percentage of patients, vague abdominal pain and poor appetite.
Patients with colonic nigromegaly usually suffer from intestinal dysfunction and electrolyte disorders due to the invasion of the intramucosal nerve plexus, which can lead to hypokalemia and hyponatremia. When hypokalemia and hyponatremia are present, in addition to gastrointestinal discomforts, they may cause cardiac discomforts such as chest tightness and palpitations, as well as generalized discomforts such as weakness, numbness of the limbs, and muscular aches and pains.
