The surgical procedure of choledochoscopic lithotripsy generally consists of three steps: incision of the bile duct or extraction of the T-tube, choledochoscopic search for stones, and stone extraction. 1. Incision of the bile duct or removal of the T-tube: general choledochoscopic stone extraction requires laparoscopic finding and incision of the common bile duct first. In case of postoperative bile duct stone extraction, it is usually done through the fibrous sinus tract formed by the pre-retained T-tube, which needs to be pulled out first. 2. Choledochoscope to find stones: The choledochoscope is slowly inserted into the common bile duct, and the images on the display screen are used to observe and search for stones, and to determine the location and distribution as well as the size of the stones. 3. Stone extraction: After finding the stone, under the direct vision of choledochoscope, the stone extraction basket is placed to the distal end of the stone, and the stone is slowly removed after opening the basket. The procedure of choledochoscopic lithotripsy is only three steps as mentioned above, but the concept of asepsis needs to be emphasized. It is recommended to perform the procedure in a regular hospital to avoid postoperative complications such as cholestatic peritonitis.
