How to effectively prevent abscesses from breaking into the chest cavity

Lung abscess chest pain in the elderly is caused by inflammatory lesions involving the pleura, and the pain is aggravated during breathing. If the abscess breaks into the chest cavity, it can form an abscess chest, pneumothorax, or an encapsulated abscess chest due to fibrin wrapping, and the patient’s respiratory movement is restricted and shortness of breath is aggravated. How to effectively prevent lung abscess? Infection foci in the upper respiratory tract and oral cavity must be eradicated. During oral surgery, the secretions should be aspirated as much as possible. Patients in coma or general anesthesia should be given intensive care to prevent lung infections. Early and thorough treatment is the key to eradicating lung abscess. The principles of treatment are anti-inflammation and drainage. 1.Antibiotics The infecting bacteria including most of the anaerobic bacteria for the treatment of acute lung abscess are sensitive to penicillin and have better efficacy, so it is most commonly used. Dose according to the condition, generally acute lung abscess can be cured by penicillin treatment. Bacteroides fragilis is not sensitive to penicillin, and can be injected intramuscularly with lincomycin; in serious cases, it can be injected intravenously. Or clindamycin can be given orally. Or metronidazole orally. Lung abscess caused by Legionella pneumophila, erythromycin treatment has good effect. In addition to systemic medication, local treatment such as cricothyroid puncture, nasal catheter endotracheal or fiberoptic bronchoscopic drip is added, and penicillin is commonly used, and after the drip, appropriate position is taken according to the abscess site and resting for 1 hour. Hematogenous lung abscess is a complication of sepsis, and should be treated as sepsis. 2.Sputum drainage Expectorants are taken orally to make sputum easy to cough up. For thick sputum, airway humidification such as steam inhalation and ultrasonic nebulized inhalation can be used to facilitate the drainage of sputum. For patients with good general condition and low fever, postural drainage can help drainage of pus. Make the abscess site in high position, tap on the affected part, and with obvious signs of sputum obstruction, it can be flushed and attracted by fiberoptic bronchoscope. 3.Surgical treatment If bronchial obstruction is suspected to be bronchial cancer; if chronic lung abscess is treated by internal medicine for 3 months, but the abscess cavity still does not shrink and the infection cannot be controlled; or if it is complicated by bronchial dilatation, abscess chest, bronchopleural fistula; if large hemoptysis is life-threatening, surgical treatment is needed.
