Pediatric incarcerated inguinal hernia is one of the common emergencies in pediatric surgery. After entering the hernia sac of the intestinal tube incarcerated, the child feels pain and often cry, crying can make the abdominal pressure increase and aggravate the incarceration, thus appearing incarcerated intestinal blood circulation obstruction, causing venous return obstruction, intestinal stagnation, edema, and in severe cases, intestinal necrosis can occur. The key to treatment lies in early detection, timely repositioning, relief of obstruction, if not timely relief of incarceration, can develop into strangulated hernia and intestinal necrosis, causing serious consequences. Indications for maneuver reset Due to the poor development of pediatric abdominal wall muscles, the inguinal canal around the layers of tissue is relatively soft, and vascular elasticity is better, the development of local intestinal blood circulation disorders is slower, the emergence of intestinal necrosis for a longer period of time, therefore, as long as the incarceration time is not more than 24 hours, the systemic condition is better, can be tried to reset the maneuver. The indications for reset should be considered from the following aspects: 1, time of incarceration: generally should be within 24 hours, when the incarceration time is longer, due to the edema of the intestinal wall, the reset is difficult to be successful, and it is easy to cause intestinal rupture during the reset. 2, the systemic situation: the child without obvious dehydration and acidosis, feasible manipulation reset, if the above performance, then consider the possibility of intestinal necrosis, reset may be necrotic intestinal tube pushed into the abdominal cavity and cause peritonitis. 3, hernia block local situation: local skin color is normal, no obvious redness and swelling, hernia block is soft, tension is not high, and pressure pain is relatively mild, indicating that the intestinal tube is not necrotic, the hernia sac seepage is not much, the reset is easy to succeed. 4, abdominal condition: no obvious abdominal distension and peritoneal irritation signs, feasible manipulation reset. Some scholars believe that the incarceration time is not an absolute indication of manipulation, such as hernia block tension is not big, the scrotum is not edema and redness, although the time is a little long, can still try manipulation reset.
