Differential Diagnosis of Decreased Corneal Perception

Decreased corneal perception causes a reduction in transients and affects tear film reconstruction. On the one hand, corneal hyperalgesia reduces the nerve impulses transmitted from the cornea to the brain system via the reflex arc, leading to a reduction in the nerve impulses transmitted from the brain down to the lacrimal glands and causing a reduction in the basal secretion of tears. On the other hand, after corneal hyperalgesia, since the transient frequency is positively correlated with corneal perception, the corresponding transient frequency will be reduced, and transient is the basis of tear film reconstruction: every time you blink, the transient action distributes the tear mucin uniformly on the surface of the cornea, and also the aqueous and lipid layers uniformly on the surface of the cornea, completing the reconstruction of the tear film. Reduced blinking will lead to enhanced evaporation of tears and affect the uniform distribution of mucin on the ocular surface, thus preventing the aqueous and lipid layers from adhering well and affecting tear film reconstruction. Decreased corneal perception is a clinical sign of exposure keratitis. Exposure keratitis is common in various lesions of incomplete lid closure, resulting in corneal exposure and impaired transient eye movements, and corneal epithelial damage that occurs when tears fail to wet the cornea properly. What are the symptoms that are easily confused with it? 1, prostrate keratitis is mainly caused by bacteria with high virulence. Pneumococcus, hemolytic or green streptococcus, gonococcus, bacillus subtilis, staphylococcus aureus and so on can cause disease. The onset of the disease is often preceded by a history of trauma to the corneal surface, or dust and other foreign objects into the eye. Systemic malnutrition, weakness is also easy to develop; chronic dacryocystitis patients are prone to this disease. 2, Pseudomonas aeruginosa keratitis This disease is caused by Pseudomonas aeruginosa direct invasion of the cornea caused by infection. Pseudomonas aeruginosa virulence is very strong, but the invasive force is very weak, it must be broken through the epithelium to invade the corneal tissue causing infection. Therefore, all kinds of corneal trauma, keratitis, corneal chondromalacia, etc. can be the trigger of infection. Patients with this disease are extremely uncomfortable due to corneal exposure. It is important to prevent corneal dryness at all times while treating the primary disease, e.g., before sleeping, make sure to apply a large amount of antimicrobial eye ointment to prevent corneal dryness and infection. If necessary, the doctor can temporarily suture the eyelid, leaving a slit in the center of the lid to see, but also keeping the corneal conjunctiva moist.
