Is it just insomnia?

  When she entered the clinic, the patient was very irritable and easily provoked, and she was disgusted with her family for sending her to the psychosomatic department. Her answer was “insomnia”, which had been causing her extreme pain for nearly three months, so that she wanted to end her life. Among my patients, there is no shortage of patients with negative thoughts and behaviors, but it is relatively rare for patients to commit suicide because of “insomnia”.  Follow up the medical history: the patient is usually introverted, rigorous, few interests, retired two years, but just retired did not leave the post, was rehired by the unit to continue to work, January 2008 began to officially leave work at home. Her husband has a good monthly income from business, but he is busy working and socializing a lot, her son is newly married for one year after graduating from college, and lives separately from her parents, and her household chores are undertaken by a nanny. After she left her job, she spent most of her time at home alone, often unable to find anyone to talk to for a few days, gradually developed insomnia symptoms, mainly difficulty in sleeping, thoughtfulness before going to bed, past events from decades ago came to mind and could not be controlled, at the same time, her personality became radical, fussy, dissatisfied with everything, often lost her temper over trivial matters, making her family feel at a loss as to how to get along with her, her son and daughter-in-law also found excuses not to come home, the original love to watch I can’t concentrate on the TV series I used to love. He bought some sleeping pills at the pharmacy and they were ineffective, so his insomnia became more and more serious at night, and sometimes he stayed up all night for several nights, so he was in extreme mental pain that he chose to commit suicide to free himself.  The patient appears to be suicidal due to “insomnia”, but the real reason is an emotional reaction, an anxiety reaction due to the change of the original living environment and the inability to adapt to the new environment, and insomnia is only one of the symptoms. The problem can not be solved by improving insomnia.  1. Psychological guidance: Give the patient a patient explanation and tell her why she has insomnia. Why has her personality changed so much? Understand her mental pain and sympathize with her current situation. At the same time, she was also given appropriate reassurance, telling her that there were many patients similar to her, and that her sleep could be improved after improving her anxiety. The patient expressed great gratitude and confidence in the treatment.  2.Medication: She was given anti-anxiety and sleep improvement medication, and was instructed to follow up with me for two weeks, and her cell phone number was given to her to facilitate her contact with me at any time, so that she could take the medication with confidence and increase her compliance with the treatment.  3. Family therapy: The patient had not found a suitable lifestyle after retirement, and her loneliness, isolation and emptiness made her psychologically drifted, and the affection of her husband and son could help her get through this difficult period. Communicate with her husband and son, agree to have family meals together at least twice a week, and usually show more care and communication to the patient as much as possible.  4. Adapt to the new environment: Instruct the patient to enjoy life and establish new social interpersonal relationships. She was advised to swim for one hour every day; to join the community drum team; to learn a musical instrument at the university for the elderly, etc.  The patient and her family were satisfied with the treatment effect and the quality of life improved significantly. In the future, the patient had regular monthly follow-ups and stopped the medication after six months.  Conclusion: Insomnia is a common symptom, especially in the elderly, but some mood disorders are also accompanied by insomnia symptoms, such as anxiety and depression, etc. It is ineffective to improve sleep alone, so detailed medical history, medication, psychotherapy, family therapy, etc. are carried out at the same time, and help them solve some problems in environmental adaptation, marriage, interpersonal relationships, etc. as much as possible, otherwise the treatment is not effective, and There may be serious consequences such as patient suicide.
