How to be alerted to nocturnal wheezing and telescopic breathing

In life, we may encounter family members or friends who experience nocturnal wheezing and need to sit up for slight relief. Such patients often wake up suddenly with shortness of breath while sleeping at night, accompanied by discomfort such as inability to lie down, weakness, coughing and coughing, and panic. At this time, family members who do not understand the situation may mistakenly believe that the cough is caused by poor coughing and phlegm blocking the airway, and give the patient some cough and phlegm medicines to relieve the symptoms. In fact, it may not be possible to solve the problem at the root. You should go to the hospital promptly and let a specialist diagnose and treat the problem. You may say, “Isn’t it just a cough and shortness of breath, a respiratory problem? Or you may not pay attention to it because it can be relieved by itself. However, this may not be the case. Although it can also be caused by respiratory disease, telangiectatic breathing often suggests that the patient may have left heart failure. Chronic heart failure is the ultimate destination of most cardiovascular diseases and the leading cause of death. How is it related to the heart when it is clearly dyspnea and coughing? First of all, the anatomical position is such that venous blood comes back from the body circulation through the pulmonary artery, blood is exchanged in the lungs, carbon dioxide is eliminated, fresh oxygen is taken in, oxygen-rich blood is remitted to the left atrium through the pulmonary veins, and then from the left atrium into the left ventricle, called the pulmonary circulation. When the left heart fails, its function decreases and the blood that should have entered the atrium from the pulmonary vein is blocked, so stasis in the pulmonary circulation occurs. In turn, respiratory symptoms such as wheezing and coughing occur. Why is it mostly aggravated at night when lying down? The mechanism is complex, but it is simply related to the increase in pulmonary blood volume due to redistribution of blood when lying down, increased vagal tone at night, constriction of small bronchi, high septum, and reduced lung volume. For those who experience nocturnal wheezing, it is important to visit the hospital in a timely manner, and for those who are diagnosed with left heart failure, do not be overly worried and follow the doctor’s treatment plan for standardization. Of course, it is essential to improve your lifestyle. The first is diet, a small number of meals to eat easily digestible light and low-salt food, elderly patients with heart failure mostly have abdominal distension, abdominal pain, nausea, vomiting and other symptoms, which is related to liver and gastrointestinal stasis, so they eat less, nutritional intake is not enough, so it is more important to pay attention to nutrition, to ensure adequate protein and potassium intake, salt intake is controlled at 2-3g/day. Patients with heart failure often have an obvious sense of thirst, but still should control water intake, about 1.5-2.0L/day. In terms of exercise, heart failure patients mostly have fatigue and weakness, so they are reluctant to be active, however, prolonged rest can cause thromboembolic disease, joint muscle stiffness and atrophy, as well as urinary difficulties and other comorbidities, which can easily lead to cardiac neurosis, which is more detrimental to recovery. Studies have shown that patients with heart failure can benefit from appropriate exercise. Patients with stable heart failure and good cardiac function can take appropriate walks in the company of family members. Patients with severe heart failure, on the other hand, can practice sitting and standing at the bedside and walking for five minutes twice daily when their condition improves. The disease itself is painful, so it needs the patience of family members, and patients themselves should have the confidence to overcome the disease, take medication on time, actively improve their lifestyle, and improve their quality of life.
