What is “partial closure”?

  ”The basic operation is to inject a mixture of local anesthetics and hormonal drugs into the painful area to achieve the purpose of anti-inflammation and analgesia. This is also how local closure therapy evolved in dermatology, and is very effective in treating keloids and baldness. Today we will briefly introduce the local seal treatment of keloids.  A keloid is an excessive tissue reaction to trauma in the dermis, characterized by proliferation of dermal fibroblasts and production of excessive extracellular matrix, and changes in the quality and quantity of fibroblasts, which are the main factors in the formation of keloid scars. The most commonly used drugs for the treatment of keloid scars are Depo-Provera and Tretinoin. The mechanism of action of these two drugs is similar, both of them use hormones to reduce the accumulation of leukocytes at the inflammation site, inhibit capillary proliferation, reduce capillary permeability and edema formation; they have antagonistic effects on kinins and prostaglandins, inhibit inflammatory reactions, anti-allergic and immunosuppressive effects. Local seal treatment has rapid efficacy and absorption; meanwhile, if long-acting hormones are used, the efficacy can be stored and maintained to achieve longer-term symptom control.  Of course, after a large number of long-term clinical cases, it is found that although the local sealing treatment with hormone drugs alone is effective, there are also rebound situations. To address this shortcoming, the clinical need to be combined with other drugs to consolidate treatment. One study showed that the combination of Depo-Provera with Xylazine increased the efficiency of keloid treatment by 24.2%. The combination of hormonal local seal treatment with cryotherapy significantly improves the efficiency and healing rate. For larger keloids, CO2 laser surgery can also be combined with the removal of the scar with CO2 laser first, and drug closure treatment is more effective after the wound has been debrided and healed.
