Abstract: Ms. Li, 32 years old, usually has no oral discomfort symptoms, but I don’t know from which day she started to bleed when she brushes her gums and when she bites harder foods such as apples. Upon careful examination of Ms. Li’s mouth, the gums became red and swollen, and calculus was visible. Gingivitis was considered to be the cause of Ms. Li’s bleeding gums. After scaling + medication, the gum bleeding and redness disappeared and the treatment effect was good.

I. Initial consultation
Ms. Li came to our hospital in May 2022 and reported that her teeth were bleeding when she brushed them for a period of time, and whenever she ate something hard, such as an apple, she would bleed when she bit into it. On examination, Ms. Li’s oral hygiene was general, her mandibular teeth were crowded, calculus could be seen on the lingual side of the lower anterior teeth and the buccal side of the bilateral molars, and the gingival papillae were rounded and hypertrophied, with disappearance of point color and a bright surface. The gingival texture was soft and fragile and lacked elasticity. Ms. Li denied the history of systemic diseases such as hypertension, diabetes, heart disease, etc. She had no history of smoking or drug allergy, and her brushing habit was 2 times/day for 2 minutes each time. Because Ms. Li’s gums showed congestion and swelling with bright red color, bleeding was detected. Based on Ms. Li’s clinical presentation, medical history and findings, gingivitis was diagnosed.

II. Treatment history
After completing liver function and other related tests, blood diseases and hepatitis were excluded. According to Ms. Li’s symptoms, physical examination results and daily living habits, the initial consideration was gingivitis caused by inadequate oral cleaning and misalignment of teeth. After communicating with Ms. Li about her condition and treatment plan, Ms. Li expressed her willingness to actively cooperate with the treatment, so she was given ultrasonic scaling of the whole mouth teeth, together with medication, including rinsing with hydrogen peroxide solution and topical application of iodine glycerin after scaling. During the treatment period, Ms. Li was advised to eat a light diet.
III. Treatment effect
After Ms. Li underwent ultrasonic cleansing and drug treatment, the swelling symptoms were relieved and the bleeding symptoms during diet were significantly reduced. After 1 week of outpatient treatment, when Ms. Li came to the hospital for a review, she could see that the redness and swelling of the previously congested gums in her mouth had decreased and the color had changed from bright red to light pink, so the treatment effect was good. Ms. Li reported that she felt good and her diet gradually returned to normal, so she was allowed to stop the medication. At the same time, Ms. Li was told to continue to eat a light diet and to come to the hospital for immediate review if her gums became red, swollen and painful again. In addition, Ms. Li was advised to undergo full-mouth orthodontic treatment to align her teeth, which she expressed willingness to consider.

We are glad that Ms. Li’s gum discomfort has gradually decreased and her quality of life has improved after the active treatment. However, Ms. Li needs to note that although her gum bleeding symptoms have greatly improved after treatment, she should still develop good brushing habits and have regular dental checkups to avoid the accumulation of plaque and calculus in her mouth for a long time, which may cause diseases.
