High risk factors for recurrence include: histologic hypofractionation (except for MSI-H-like tumors), lymphovascular/vascular invasion, intestinal obstruction, <12 lymph nodes sent for examination, nerve invasion, limited bowel perforation, or close, indeterminate, or positive perineural infiltration (PNI) - the presence of PNI is associated with a significant poor prognosis. In a multifactorial analysis, PNI was found to be an independent poor prognostic factor, both in terms of cancer-specific and overall disease-free survival. For stage II bowel cancer, patients with PNI had a significantly worse prognosis than those without PNI, with a 5-year DFS of 29% versus 82%, respectively.
