Stomach pain during menstruation is considered dysmenorrhea in traditional Chinese medicine, the medication and treatment need to identify the type, usually can be divided into the following five types of evidence:
1. Liver and kidney deficiency syndrome: manifested as vague pain in the abdomen during menstruation, pale color and thinness, dizziness and tinnitus, lumbago and leg weakness, like to press, low menstrual flow, long and clear urine, and dull complexion. Treatment is recommended to nourish the liver and kidney, regulate menstruation and relieve pain, using Yi Kidney Regulating Menstruation Soup for treatment. Contraindications are not clear.
2. Weakness of qi and blood: symptoms include dizziness and palpitations during menstruation (rapid heartbeat, often accompanied by panic), low menstrual flow, insomnia and dreaminess, vague pain in the abdomen and liking to be pressed, light color and thin texture, fatigue (mental exhaustion, physical weakness), and pale complexion. Treatment should be to replenish Qi and nourish blood, harmonize the middle and relieve pain, using Huang Qi Jian Zhong Tang plus subtractions. Avoid seaweed, woad and raw onion.
3. Qi stagnation and blood stasis type: menstruation with purplish color and lumps, reduced pain under the lumps, pre-menstrual abdominal distension and pain refusing to be pressed, irregular menstruation, distension and pain in the chest and ribs, and breasts. The treatment is to move Qi and activate blood circulation (to promote the flow of Qi and blood circulation), dispel blood stasis and relieve pain (to remove blood stasis to relieve pain), and use Diaphragm Downward Expelling Blood Stasis Tang. Contraindications are not clear.
4. Cold-congealed blood stasis type: cold pain in the abdomen before menstruation and refusing to be pressed, small amount of menstrual blood, dull color with lumps, fear of cold and cold limbs (fear of cold, cold limbs), pain decreases when there is heat, and the color of the face is greenish-white. Warm menstruation soup is recommended, and avoid eating cold and raw food during the period of taking the medicine.
5. Damp-heat accumulation type: pain intensifies before menstruation, heavy menstrual flow or long menstrual period, burning pain in the abdomen before menstruation and refusing to be pressed, purplish-red menstruation, thick or clotted, pain connecting the lumbar and sacral regions, or usual pain in the abdomen, and a lot of yellow, thick and foul-smelling discharge in normal life. The choice of clearing heat and regulating blood soup plus subtractions, qi and blood weakness and abdominal pain can not be used.
