Abstract: A 50-year-old middle-aged male patient with unexplained shoulder pain and limited shoulder movement came to our hospital. After examination, he was diagnosed with adhesive capsulitis, also known as frozen shoulder or frozen shoulder, and through oral medication, together with rehabilitation treatment (passive shoulder arthrodesis, active wall climbing, attachment ring rotation of the shoulder, etc.), the patient’s shoulder pain symptoms were significantly relieved and he has now resumed normal shoulder movement.

I. Initial consultation
The patient was a 50-year-old male patient who came to our hospital because of shoulder pain for several months and the degree of shoulder pain was affecting his sleep. After careful questioning, the patient said, “Four months ago, he had shoulder pain in a fixed position without any obvious cause, and he felt obvious dull pain and discomfort in the shoulder joint, and could not take care of himself. On examination, the patient had limited active and passive shoulder joint movement and extensive pressure pain in the shoulder, and X-ray examination showed degenerative changes in the shoulder joint.
II. Treatment history
The patient reported that he was afraid of heat, liked to blow cold air against the air conditioner, liked to keep his shoulder exposed for a long time, and did not like to move around. The patient was recommended to be treated conservatively with Celecoxib capsules for anti-inflammatory and analgesic treatment, together with Myclobutanil tablets for oral swelling reduction. At the same time, the patient underwent shoulder arthrodesis and was encouraged to take the initiative to perform wall climbing and attached circular shoulder exercise with the cooperation of medication, together with shock wave therapy for further anti-inflammatory and pain relief.
III. Treatment effect
The patient actively cooperated with the treatment methods and active exercises provided by us. After about 2 weeks of treatment, the patient’s shoulder pain symptoms improved and he could resume normal sleep; after 1 month of follow-up, the patient’s shoulder pain symptoms had disappeared, the shoulder joint resumed normal activities such as internal rotation and abduction, and basically resumed self-care ability, and the quality of sleep was greatly improved, and the patient was very satisfied with the effect of this treatment.
IV. Notes
I am glad that the patient’s symptoms were relieved, but I would like to advise the patient to avoid fixing the shoulder in one position for a long time in daily life, and to pay attention to regular exercise of the shoulder area. In addition, it is necessary to pay attention to the back of the shoulder to keep it warm and avoid direct contact with cold wind to avoid the recurrence of symptoms. At the same time, you should pay attention to observe whether there is any other discomfort in the shoulder area, and if you feel any symptoms such as shoulder pain and restricted activity, you should go to the hospital for a review and take timely treatment measures.
V. Personal insight
Adhesive shoulder capsulitis occurs in middle-aged and elderly people, like this patient who likes to blow air conditioner for a long time, so that the back of the shoulder is attacked by cold wind, and at the same time does not pay attention to shoulder joint exercise, which can easily cause pathological changes in the shoulder joint, and then cause obvious pain and limited activities in the shoulder. Therefore, middle-aged and elderly people should pay attention to protect the back of the shoulder, pay attention to local warmth, and participate in more exercises, which can effectively prevent the occurrence of adhesive capsulitis.
