In some patients, after surgery, the fistula recurs after a period of time, with the same painful swelling and pus as before. The reason for recurrence after anal fistula surgery is that more than 95% of the cases are due to the fact that the internal opening was not found accurately, or the primary site of infection, the infected anal sinus, the ducts and glands of the anal glands, and the branches and cavities of the fistula were not completely and thoroughly removed. ” Because the anal sinus is very small, the ducts and glands of the anal glands are curved, some upward and some downward, and widely distributed between the sphincter spaces, the infection can spread around and form a complex situation with multiple branches of dead cavities. The surgery is not an easy task to find the original orifice, especially if there are more than two orifices and the location is high, it is even more difficult to find the orifice, or in the search, artificially poke out an orifice with a probe, the wound will not heal after the surgery, or the fistula will break and flow again from time to time. The author has seen patients with anal fistulas who have had five surgeries. According to the follow-up statistics of 1246 anal fistula surgeries in our hospital in 10 years, there were 61 cases of recurrence, with a recurrence rate of 4.89%. The main reasons for this are points 1 and 2, one is that some fistulas have multiple internal openings or are located too high to detect deviations and are not treated thoroughly, and the other is that fistula surgery in our hospital is generally not inpatient, some patients do not realize the importance of post-operative medication changes, some patients are too busy to leave because of work, and some patients do not come to the hospital for follow-up as required because of the inconvenient distance. The change of medication leads to recurrence. The actual procedure will be a good one.
